Item No.: 08
Site: Potable Water-Medical Exam Rooms
Violation: The three medical exam room showers had Watts N9 backflow prevention devices. The cross-connection program list indicated that the showers were equipped with Oras backflow prevention devices. In addition, the backflow prevention device on the spray hose in the doctor's toilet was not on the cross-connection program list.
Recommendation: Ensure the cross-connection control program listing of backflow prevention devices is complete and accurate.

Site: Potable Water-Engine Room Bunkered Water Halogen Injection
Violation: The cross-connection control program did not list the air gaps for the chemical vat fill lines. The hose connected to the fill line on the right-side chlorine vat fill line was too long and could be put into the vat.
Recommendation: Ensure the cross-conection control program lists air gaps.

Violation: The men's and women's shower backflow prevention devices were not on the cross-connection control program list.

Violation: The make-up water lines were equipped with Watts 9D backflow prevention devices. The cross-connection control program listing indicated that the lines were protected wtih Watts N9 back flow prevention devices.
Recommendation: Ensure teh cross-connection control program backflow prevention device listing is complete and accurate.

Violation: There was no indication in the records from 4/2/2009 to the current date that the whirlpool spa water was changed daily.
Recommendation: Ensure that the whirlpool spa water is changed daily.

Site: Swimming Pool-Kid's Pool - Logs
Violation: On 5/26/2009 the kid's pool log indicated that the kid's pool was closed at 1800 for a vomit incident. The fecal accident log indicated that the kid's pool was closed at 1200 for a fecal accident.
Recommendation: Ensure that a written or electronic record is made of all fecal accidents. Ensure that the record includes the pool name, date and time of incident, response steps taken, the free residual halogen level achieved following cleaning, and the contact time.

Site: Swimming Pool-Main Pool - Logs
Violation: On 3/3/2009 the fecal accident log indicated that the main pool was closed for a vomit incident. The pool log indicated that the main pool was open. On 3/28/2009 the pool log indicated that the Main pool was closed at 1300 for a vomit incident. The fecal accident log indicated that the Main pool was closed at 1130 for a fecal accident.
Recommendation: Ensure logs accurately reflect pool closure and fecal/vomit accident responses.

Violation: There were some missing fasteners in the water contact zone of the ice machine adjacent to reach-in refrigerator # L6F.
Recommendation: Replace the missing fasteners.

Site: Galley-Beverage Station (Forward)
Violation: There was peeling sealant between the ice cuber and ice bin of the aft ice machine.
Recommendation: Ensure that multiuse food-contact surfaces are: (1) Smooth; (2) Free of breaks, open seams >1 mm (1/32 inch), cracks, chips, inclusions, pits, and similar imperfections; (3) Free of sharp internal angles, corners, and crevices; (4) Finished to have smooth welds and joints; and (5) Accessible for cleaning and inspection by one of the following methods without being disassembled, by disassembling without the use of tools, or by easy disassembling with the use of handheld tools commonly available to maintenance and cleaning personnel such as screwdrivers, pliers, open-end wrenches, and Allen wrenches.

Violation: One of the two upper final rinse spray arms on the in-use conveyor dishwasher was not working. This item was corrected by the end of the inspection.
Recommendation: Ensure that warewashing equipment is maintained in good repair and proper adjustment including: (1) Being maintained in a state of repair and condition that meets the standards of the materials, design, and construction of these guidelines; and (2) Maintaining water pressure, and water temperature measuring devices in good repair and ensuring their accuracy within the intended range of use.

Site: Galley-Lido Center
Violation: The in-use conveyor dishwasher registered 154°F twice at the plate surface using the inspector's thermometer and 155°F using the ship's maximum-registering thermometer. In addition, the thermometers were held in the final rinse spray and registered 155°F. There was insufficient hot water sanitizing at the plate/utensil surface.
Recommendation: In a mechanical operation, ensure that the temperature of the fresh hot water sanitizing rinse as it enters the manifold is not more than 90°C (194°F), or less than: (1) For a stationary rack, single temperature machine, 74°C (165°F); or (2) For all other machines, 82°C (180°F); or (3) A utensil surface temperature of 71°C (160°F) as measured by an irreversible registering temperature indicator shall be achieved. (4) The maximum temperature of 90°C (194°F), does not apply to the high pressure and temperature systems with wand-type, hand-held, spraying devices used for the in-place cleaning and sanitizing of equipment such as meat saws.

Violation: The concentration of halogen in the sanitizer container was well above 200 ppm. This was corrected.
Recommendation: Ensure that a chlorine solution used as a sanitizing solution has a concentration between 50 mg/L (ppm) and 200 mg/L (ppm).

Violation: Several coffee cups placed out for service were soiled. These items were removed and sent to the dishwasher.
Recommendation: Ensure that food-contact surfaces of equipment and utensils are clean to sight and touch.

Violation: There was a leak at the shower hose backflow prevention device. This shower was not used.

Violation: The light level along the soiled side was not adequate at any of the large waiter stations. This is scheduled for repair at the January 2010 drydock. This item is continued from the previous inspection.
Recommendation: Ensure that the light intensity is at least 220 Lux (20 foot candles) in bars and at dining room waiter stations during periods of cleaning.

Violation: The lighting at the side and back of the ice machine was less than the required 110 lux at 30 inches above the deck during periods of cleaning.
Recommendation: Ensure that the light intensity is at least 110 Lux (10 foot candles) at a distance of 75 centimeters (30 inches) above the deck when in use, in walk-in refrigerator units and dry storage areas, and in other areas and rooms during periods of cleaning.
