This cruise ship inspection report lists the deficiencies found during the inspection. Additional information corresponding to each item number is available in the lastest editions of the CDC VSP Operational Manual.



 
    Cruise Ship's Corrective Action Statement

Item No.: 08
Site: Potable Water-Cross-Connection Control Program
Violation: The cross-connection control program listed the potable water lines supplying the juice machines, coffee machines and baking equipment as being protected by Watts 9D backflow prevention devices. The connections were protected by Watts N9 devices.
Recommendation: Ensure that the cross-connection control program includes at minimum: a complete listing of cross-connections and the backflow prevention method/device for each, so that there is a match to the plumbing system component and location, and an inspection frequency. Ensure that air-gaps are included in the listing.

Site: Potable Water-Cross-Connection Control Program
Violation: The number of connections for the passenger, crew and petty officer cabins was listed. However, on some decks, the type of devices used for protection were not listed.
Recommendation: Ensure that the cross-connection control program includes at minimum: a complete listing of cross-connections and the backflow prevention method/device for each, so that there is a match to the plumbing system component and location, and an inspection frequency. Ensure that air-gaps are included in the listing.

Site: Potable Water-Testable Devices
Violation: The test results showing the pressure differentials on both sides of the valves was not recorded with the annual test. The records indicated that the tests were done per the work order description.
Recommendation: Ensure that backflow prevention devices requiring testing, for example reduced pressure backflow preventer and double check valves with test cocks, are inspected and tested with a test kit at least annually and that the test results showing the pressure differences on both sides of the valves are maintained for each device.

Site: Potable Water-Cross-Connection Control Program
Violation: The cross-connection control program listed the potable water connections to the gun mixing units. It did not list the connection for the carbonation unit itself. Inside the unit a Chudnow backflow prevention device is installed between the brass fitting of the pump and the carbonation tank.
Recommendation: Ensure that the cross-connection control program includes at minimum: a complete listing of cross-connections and the backflow prevention method/device for each, so that there is a match to the plumbing system component and location, and an inspection frequency. Ensure that air-gaps are included in the listing.

Site: Swimming Pool-Calypso
Violation: A fecal/vomit accident log was not kept by the vessel. On May 13, 2009 an accident occurred at the Calypso pool. The records indicated that the fecal/vomit accident plan was followed. The pool name, date and time of the accident, response steps taken, free residual halogen level achieved following cleaning and contact times were not recorded.
Recommendation: Ensure that a written or electronic record is made of all fecal accidents. Ensure that the record includes the pool name, date and time of incident, response steps taken, the free residual halogen level achieved following cleaning, and the contact time.

Site: Whirlpool/Spa-Calypso
Violation: The pH was below 7.2 for the entire day of March 1, 2009 at the Calypso starboard whirlpool. This also occurred at the Calypso port whirlpool on May 13, 2009.
Recommendation: Ensure that the whirlpool spa water is maintained with a pH between 7.2 and 7.8.

Violation: The whirlpools were not being drained or backwashed daily. The free residual halogen was raised to above 10 ppm at the end of the day and then the water was held until they ship was more than 12 miles from Alaska. Once the ship was 12 miles out, the filters were backwashed and the whirlpools drained.
Recommendation: Ensure that granular filters are backwashed at least daily. Repeat backwashing, if necessary, until the water viewed through the sight glass or discharge point is clean flowing.

Violation: Per staff, the cold counter top and the bain marie units were on time control in the room service area and the pass-through refrigerators were on time control in the galleys. However, these units were not listed in the time as a public health control plan.
Recommendation: Ensure the time as a public health control plan reflects the operation.

Violation: The plastic serving spoons for the condiments were not located under a sneeze guard.
Recommendation: Ensure that condiments are protected from contamination by being kept in dispensers that are designed to provide protection, protected food displays provided with the proper utensils, original containers designed for dispensing, or individual packages or portions.

Violation: The vessel was using either Dow Corning 732 or 3M DP110 epoxy on the food contact surfaces of the slicer. The Dow Corning 732 was a soft sealant. The 3M epoxy was not approved for use on food contact surfaces.
Recommendation: Ensure that a sealant approved for food contact surfaces is applied to food contact surfaces. Ensure the sealant dries to a smooth, hard finish.

Violation: The gaskets inside the chutes of the potato peelers were torn.
Recommendation: Replace the gaskets.

Recommendation: Ensure that nonfood-contact surfaces are free of unnecessary ledges, projections, and crevices, and designed and constructed to allow easy cleaning and to facilitate maintenance.

Violation: There was no conveyor speed on the data plate.
Recommendation: Ensure that warewashing machines are provided with an easily accessible and readable data plate affixed to the machine by the manufacturer that indicates the machine's design and operating specifications including the: (1) Temperatures required for washing, rinsing, and sanitizing; (2) Pressure required for the fresh water sanitizing rinse; and (3) Conveyor speed for conveyor machines or cycle time for stationary rack machines.

Violation: There was food debris inside the seam along the right edge of the induction cooktop.
Recommendation: Ensure that nonfood-contact surfaces of equipment are kept free of an accumulation of dust, dirt, food residue, and other debris.

Violation: Per staff, the technical compartments of the ice machines were only cleaned and not sanitized per manufacturer's instructions.
Recommendation: Ensure that the cleaning of equipment such as ice bins and beverage dispensing nozzles and enclosed components of equipment such as ice makers, cooking oil storage tanks, and distribution lines, beverage, and syrup dispensing lines or tubes, and coffee bean grinders is conducted: (1) At a frequency specified by the manufacturer, or (2) If manufacturer specifications are absent, at a frequency necessary to preclude accumulation of soil or mold.

Recommendation: Ensure that a sign stating "WASH HANDS OFTEN" in a language that the food employees understand is posted over handwashing sinks.

Recommendation: Ensure that the light intensity is at least 220 Lux (20 foot candles) on food preparation surfaces, and at a distance of 75 centimeters (30 inches) above the deck in food preparation areas, handwashing facilities, warewashing areas, equipment, and utensil storage, pantries, toilet rooms, and consumer self-service areas.

Site: Housekeeping-Children's Pool Restroom
Violation: There was no means to exit the children's restroom by the children's pool without barehand contact with the door.
Recommendation: Equip public toilet facilities so that persons exiting the toilet room are not required to handle the door with bare hands. This may be accomplished by methods such as locating paper towel dispensers at sinks and waste containers near the room door, installing mechanically operated doors, door removal, or other effective means.
