Item No.: 08
Site: Provisions-Trolley Wash
Violation: A hose with a nozzle/valve was attached to the mop fill station. The hose could be charged under continuous pressure. The installed vacuum breaker was not sufficient to provide backflow protection under continuous pressure.
Recommendation: Ensure that a continuous pressure-type backflow preventer is installed when a valve is located downstream from the backflow preventer.

Site: Other-Deck 4 - Garbage Room
Violation: There was a non-continuous type back flow prevention device on the bucket fill line next to the handsink. A hose was attached to the line and there was a valve downstream.
Recommendation: Ensure that a continuous pressure-type backflow preventer is installed when a valve is located downstream from the backflow preventer.

Site: Other-Deck 4 - Garbage Room
Violation: The backflow prevention device on the bin washer was not listed in the cross-connection control program.
Recommendation: Add the backflow prevention device to the cross-connection control list.

Site: Other-Deck 9/FZ 3 - Chemical Locker
Violation: The listing of cross-connections indicated that there was a Watts 288A on the chemical distribution system. Per staff, all of the chemical dispenser systems had been replaced with "Swisher" units which had an internal, manufactured air gap.
Recommendation: Include the chemical dispenser air gaps on the listing of cross-connections.

Site: Buffet-Garden Cafe - Pizza
Violation: The pizza press was heavy and could not be easily moved for cleaning.
Recommendation: Ensure that table-mounted equipment that is not easily movable is elevated on legs that provide at least a 100 mm (4-inch) clearance between the table and the equipment.

Site: Galley-La Bistro - Warewashing
Violation: The flight-type warewashing machine did not have a dataplate. This item was corrected.
Recommendation: Ensure that warewashing machines are provided with an easily accessible and readable data plate affixed to the machine by the manufacturer that indicates the machine's design and operating specifications including the: (1) Temperatures required for washing, rinsing, and sanitizing; (2) Pressure required for the fresh water sanitizing rinse; and (3) Conveyor speed for conveyor machines or cycle time for stationary rack machines.

Site: Galley-Warewashing
Violation: The final rinse temperature on the installed gauge registered 38°F. In addition, the warning light was flashing. A check of the final rinse temperature measured 162°F.
Recommendation: Ensure that warewashing equipment is maintained in good repair and proper adjustment including: (1) Being maintained in a state of repair and condition that meets the standards of the materials, design, and construction of these guidelines; and (2) Maintaining water pressure, and water temperature measuring devices in good repair and ensuring their accuracy within the intended range of use.

Site: Galley-Warewashing
Violation: The coffee lids were not taken apart prior to going through the warewash machine, and were stored with clean dishes. The interior of the lids were soiled and had an accumulation of moisture.
Recommendation: For effective cleaning and sanitizing of equipment food contact and nonfood contact surfaces, ensure that small components such as coffee urn lids are taken apart or broken down prior to warewashing.

Site: Galley-Summer Palace Storage
Violation: The handwashing sink had a leak coming from the cold water supply line.
Recommendation: Ensure that a plumbing system in a food area is maintained in good repair.

Site: Galley-Garden Cafe - Warewashing
Violation: Condensate was collecting in the ventilation hood and along the bulkhead adjacent to the flight-type warewashing machine on the soiled landing. The worker was also heavily sweating.
Recommendation: Ensure that all food preparation, warewashing, and toilet rooms have sufficient ventilation to keep them free of excessive heat, steam, condensation, vapors, obnoxious odors, smoke, and fumes.

Site: Children Area-Lego Toys
Violation: Per staff, the box of Lego toys were used earlier in the day, collected and then set aside. The Legos would then be used later in the day when children returned to the activity center. These small, mouthable pieces were not cleaned between uses.
Recommendation: Ensure that toys used in the child-activity center are maintained in a clean condition. Separate the small, mouthable toys into smaller bins. Once a bin is used it can be set aside in a "soiled" area to be cleaned at the end of the day. A clean bin can be set out for new use.
