Site: Medical-Acute Gastroenteritis (AGE) Log
Violation: When viewed on the computer, the AGE log in the electronic medical database was missing some horizontal and vertical lines between rows and columns. This created some difficulty viewing patient information on the AGE log. However, the AGE log was in the correct format when exported to an excel spreadsheet.

Site: Potable Water-Microbiologic Sampling
Violation: The ship did not have the comparator used to read the E.coli water test results. Staff stated that a new comparator had been ordered.
Recommendation: Analyze samples using a method accepted in Standard Methods for the Examination of Water and Wastewater. Operate and maintain test kits, incubators, and associated equipment in accordance with the manufacturers? specifications.

Site: Potable Water-Pipe Replacement
Violation: Between 9 December and 31 March, there were 36 different entries on the potable water pipe replacement log where a section of pipe had been replaced. The final chlorine concentration before these sections of pipe were placed into service was recorded at 3 ppm for each entry. Staff could not explain how 3 ppm was consistently recorded.
Recommendation: Maintain documentation of all inspections, maintenance, cleaning, and disinfection for 12 months and ensure it is available for review during inspections. Ensure records include a method of disinfection, concentration and contact time of the disinfectant, and a halogen value of less than or equal to 5 ppm before the tank is put back into service.

Site: Potable Water-Port Bunker Station
Violation: One potable water bunkering hose stored in the hose locker was not capped at the ends or otherwise covered.
Recommendation: Ensure potable water hoses are rolled tight with the ends capped, on reels, or on racks, or with ends coupled together and stowed in potable water hose lockers.

Site: Potable Water-Bunker/Production Dosing Area
Violation: The potable water lines suppling the hose used to dilute the chlorine and acid drums was not striped blue or blue/green/blue to indicate potable water
Recommendation: Stripe or paint potable water lines either in accordance with ISO 14726 (blue/green/blue) or blue only. Ensure no other lines have the above color designations. Stripe or paint these lines at 5 meters (15 feet) intervals and on each side of partitions, decks, and bulkheads, except where decor would be marred by such markings, including potable water supply lines in technical lockers.

Violation: The potable water lines to the spray hose and to the handwash station were not striped blue or blue/green/blue to indicate potable water.

Violation: The crew member responsible for the ice machines was observed standing on the raised deck stand and straddling the portable ice container.

Violation: The bulk milk dispensing tube was longer than 25 millimeters.
Recommendation: Cut bulk milk dispensing tubes 45 degrees on the diagonal, leaving no more than 25 mm (1 inch) protruding from the chilled dispensing head.

Violation: The wash tank of the starboard dishwash machine was soiled with food debris and paper.
Recommendation: Ensure the wash, rinse, and sanitize solutions are maintained clean.

Violation: The left trolley wash machine and the hood-type potwash machine were out of service since the day prior to the inspection.
Recommendation: Maintain warewashing equipment in good repair and proper adjustment, including: (1) warewashing equipment is maintained in a state of repair and condition that meets the standards of the materials, design, and construction of these guidelines.

Violation: The crew member working on the sanitizing compartment of the three-compartment sink placed two tightly stacked pans into the sanitizing compartment and removed them also while stacked.
Recommendation: Separate food equipment to achieve complete sanitization.

Violation: The paper towel dispenser was located above the drinking fountain. After washing hands, the user could drip water onto the clean equipment when reaching for a towel.

Violation: At the Sandwich Station, the deckhead was damaged in front of the four flat grills. Staff explained the rolling shutter was broken and when attempting to fix the chain, the door hit the deckhead.
Recommendation: Ensure decks, bulkheads, and deckheads in food preparation, warewashing, pantries, bars, and food and equipment storage areas are constructed and maintained for easy cleaning. Ensure decks, bulkheads, and deckheads in food preparation, warewashing, pantries, and storage areas are maintained in good repair.

Violation: The utility sink faucet was continuously leaking.
Recommendation: Ensure a plumbing system in a food area is maintained in good repair.

Violation: The maximum light intensity was less than 220 lux at the preparation counters. It appeared some deckhead light fixtures needed to be repositioned.
Recommendation: Ensure the light intensity is at least 110 lux (10 foot candles) at handwashing stations in bars. In bars and dining room waiter stations, provide 220 lux (20 foot candles) light intensity during cleaning operations.

Site: Integrated Pest Management-Active Inspections
Violation: Since 14 December, the weekly active inspections for the Luminae, Bistro on 5, Ocean Liner, Blu Restaurant, and Tuscan Grill were all conducted at the same time. There was no variation between the night and day inspections.
Recommendation: Ensure the IPM Plan sets a schedule for periodic active monitoring inspections, including some at night or during periods of no or minimal activity.

Site: Ventilation-Air Handling Units
Violation: A total of 7 air handling units between Deck 3, Deck 6 (Room 460), and Deck 8 (Room 780) were heavily soiled with a black and white substance that appeared to be mold. The black and white substance was all over the removable condensation separators as well as the interior top, bottom, and side walls of condensation compartment in each air handling unit.
Recommendation: Keep air handling units clean.
