Site: Pantry-Forward Housekeeping Pantries B707B and B1007
Violation: Plastic food-contact components of the ice machines around the evaporators were soiled with a light-brown, rust-like liquid debris. Water for ice flowed over some of these areas. When the inspector wiped the areas with an alcohol pad, the debris transferred to the pad. This was noted in the Deck 7 Forward Pantry B707B and the Deck 10 Forward Pantry B1007.
Recommendation: Ensure food only contacts surfaces of equipment and utensils that are cleaned and sanitized. Ensure food contact surfaces of equipment and utensils are clean to sight and touch.

Site: Potable Water-Production
Violation: The ship produced potable water while sailing in the Amazon River each day and/or night from 6 March 2019 - 14 March 2019. The cruise began in Ft. Lauderdale, FL on 4 January and ended in Ft. Lauderdale, FL on 25 March 2019. This was written on the previous inspection report from December 2018.
Recommendation: Operate a reverse osmosis unit, distillation plant, or other process that supplies water to the vessel?s potable water system only while the vessel is making way, and do not operate in polluted areas, harbors, or at anchor.

Site: Potable Water-Distribution Chemical Injection Dosing Points
Violation: The halogen and pH dosing points were installed on the suction side of the delivery pumps and below the chemical dosing tanks. This was written on the previous inspection report from December 2018 and is a new requirement in the VSP 2018 Operations Manual. Crew reported this will be corrected by the new owner when the vessel goes to dry dock in July 2019. Crew reported this was part of the sale agreement for the vessel.
Recommendation: Install a check valve or nonreturn valve between the distribution halogen and pH pumps and the injection points. In addition, ensure the potable water distribution halogenation and pH chemical injection dosing points are located on the delivery line downstream of the potable water pumps, OR if the injection dosing point is before the potable water pumps, ensure it is located above the chemical dosing tanks.

Site: Recreational Water Facilities-Fecal/Vomit Accident Plan
Violation: The plan did not include the information that the water inside the recreational water facility must be maintained at a temperature of 77F (25C) or above or that the pH must be maintained at 7.5 or below during disinfection. This was written on the previous inspection report from December 2018 and is a new requirement of the VSP 2018 Operations Manual.
Recommendation: Ensure a fecal and vomit accident response procedure that meets or exceeds the procedure provided in Annex 13.9 is available for review during inspections.

Violation: The reduced pressure principle assembly backflow prevention device on the overboard discharge line was slowly, continuously dripping water.

Site: Potable Water-Cross-Connection Control Program Log
Violation: The hose bib vacuum breaker attached to the shower hose in the deck 4 hospital ward was not listed on the log.
Recommendation: Ensure the cross-connection control program includes at a minimum: a complete listing of cross-connections and the backflow prevention method or device for each, so there is a match to the plumbing system component and location. Include air gaps on the listing. Ensure a schedule for inspection frequency is set. Maintain a log documenting the inspection and maintenance in written or electronic form, and ensure it is available for review during inspections. Inspect backflow prevention devices periodically and replace any failed units. Complete a visual check for all non-testable backflow prevention devices and air gaps at least annually.

Site: Recreational Water Facilities-Starboard Whirlpool Spa Aft Records
Violation: The chart records indicated that the whirlpool spa was drained on 24 January at 2215 but was open every day until 30 January at 2200. Records indicated the facility was not drained until 31 January at 1630, and was not open on 31 January. Crew reported this was a clerical error and that the facility had been drained. The bridge log book indicated that all recreational water facilities were drained on 27 January.
Recommendation: Change the whirlpool spa water, including compensation tank, filter housing, and associated piping, every 72 hours, provided that the system is operated continuously and that the correct water chemistry levels are maintained during that period, including daily shock halogenation. Record the date and time of whirlpool spa water changes in the log. Maintain accurate documentation of the draining of whirlpool spas.

Site: Ventilation-AC Stations 8 and 10
Violation: A small amount of white debris was noted on the droplet collector inside AC 8. A larger amount of brown debris was noted on the droplet collected inside AC 10. Crew reported the debris was due to the cleaning chemicals used.
Recommendation: Keep air handling units clean.

Site: Buffet-Deck 11 Lido Pool Deck Smoking Area Storage Lockers
Violation: Four ice buckets, two blender lids, one punch bowl, and a variety of eating utensils were stored inside two wooden storage lockers. All of these items were previously cleaned. Additionally, one bottle of an unidentified chemical cleaner and one plastic container of silver polish were stored next to the punch bowl and eating utensils. This locker was not designed for food equipment storage. The locker was cleaned immediately.
Recommendation: Store cleaned equipment and utensils, laundered linens, and single-service and single-use articles: (1) in a clean, dry location; (2) in a location where they are not exposed to splash, dust, or other contamination.

Site: Buffet-Deck 11 Lido Pool Deck Smoking Area Storage Lockers
Violation: One bottle of an unidentified chemical cleaner and one plastic container of silver polish were stored next to a punch bowl and eating utensils inside two wooden storage lockers.
Recommendation: Ensure working containers used for storing poisonous or toxic materials such as cleaners and sanitizers taken from bulk supplies are clearly and individually identified with the common name of the material. Store poisonous or toxic materials used in the cleaning and maintenance of food areas in a cleaning materials locker so they cannot contaminate food, equipment, utensils, linens, and single-service and single-use articles.

Violation: The backflow prevention device for the coffee dispenser was corroded and dripping water continuously. Remediation began immediately.

Violation: A sticky residue was on the stainless-steel backsplash immediately behind the ice cream machines. Dirt and dust had also collected on the sticky residue. The inspector used an alcohol pad to remove the soiled residue. This area was previously cleaned and not in service at the time of the inspection. Cleaning began immediately.

Violation: The chlorine concentration in the sanitizing bucket was less than 50 ppm. The bucket was removed.

Violation: Difficult-to-clean holes were observed in the faceplate next to the cabinet doors between undercounter dry food cabinets DS-11D and E.
Recommendation: Ensure nonfood-contact surfaces of equipment that are exposed to splash, spillage, or other food soiling or that require frequent cleaning are constructed of a corrosion-resistant, nonabsorbent, and smooth material. Ensure nonfood-contact surfaces are free of unnecessary ledges, projections, and crevices, and designed and constructed to allow easy cleaning and to facilitate maintenance.

Violation: The bulkheads and deckhead behind this area had multiple holes where equipment for lighting were previously mounted, making these areas difficult to clean. Additionally, several deckhead tiles in this area were sagging and the exposed edges were soiled.
Recommendation: Ensure decks, bulkheads, and deckheads in food preparation, warewashing, pantries, bars, and food and equipment storage areas are constructed and maintained for easy cleaning. Violation: The countertop under the toaster was heavily soiled with bread crumbs. Additionally, the removable toaster was heavily soiled with bread crumbs inside and on top. Staff stated this area was previously cleaned and the toaster should have been cleaned prior as well. The toaster was removed for cleaning and the counter top was cleaned immediately.

Violation: The gaskets on the inside of both milk dispenser doors were heavily soiled with a black residue. The inspector used an alcohol pad to remove the residue. The area was not in use at the time of the inspection and was previously cleaned.

Violation: The door handle for the portside milk dispenser was broken, which prevented the door from closing properly. No milk was in the unit at the time of the inspection. The handle was fixed immediately.

Violation: The technical space next to dry storage locker #3 had a pipe exiting the locker and continuing through to another side of the space. The hole in the locker bulkhead did not have a flange. Additionally, the area created by the hole was excessively soiled with greater than a day's accumulation of grease and debris.

Violation: Five dead flies were in the shield of a bulkhead-mounted light above the room service counter. This area was not in service at the time of the inspection.
Recommendation: Inspect all food areas at a frequency that can quickly detect the evidence of pests, harborage conditions, cleanliness, and protection of outer openings. Remove dead or trapped insects, rodents, and other pests from control devices and the vessel at a frequency that prevents their accumulation or decomposition or the attraction of other pests.

Site: Preparation Room-Deck 4 Fish
Violation: The light intensity inside refrigerator #34 was measured at 84 lux on both sides of the cold unit. Several bags of frozen fish were stored in the unit at the time of the inspection.
Recommendation: Ensure the light intensity is at least 110 lux (10 foot candles) at a distance of 75 centimeters (30 inches) above the deck when in use, in walk-in refrigerator units and dry storage areas, and in other areas and rooms during periods of cleaning. Ensure the light intensity is at least 110 lux (10 foot candles) behind and around mounted equipment, including counter-mounted equipment.
