Abstract: This report provides a detailed description of the cognitive research design, and reports results on an item-by-item basis for the entire questionnaire. It also includes recommendations and suggestions for further research. Seven persons participated in this study and were asked to answer questions from either the original questionnaire or the revised version. The report attaches the 1990 questionnaire.
