To receive a California CLS certificate you
must:
- Obtain the interactive educational CD-ROM. CDs are available free
of charge from the Association of Public Health Laboratories. Send an
Email request to ast@aphl.org or phone 202-822-5227.
- Register for the program on the CDC PHTN website,
http://www.phppo.cdc.gov/phtnonline
- Complete one or more modules (1,2,3,or 4), then do the
accompanying post test(s) and evaluation(s).
- When prompted for type of credit, select continuing education
units (CEU).
- Print the CDC continuing education certificate.
- Send a copy of the certificate to:
