• Limited quantities of the following publications are available free of charge.
• Please enter the number of copies you would like to receive for each item. The request limit is next to each box. To see an enlarged copy of the front cover of each material, click on the photo.
• Please double check your address! Orders are not sent if address is incorrect.
• Multiple orders for the same publication to the same address will be canceled. Only the first order will be processed.
• All orders require 4 to 6 weeks for delivery. Rush orders are not available.
• Call 1 800-CDC-INFO if you require more than the maximum quantity allowed for each publication, stating the quantity requested, the purpose for which they will be used, and your contact information.
