Terms of Use

• Limited quantities of the following publications are available free of charge. You may order more later.
• Enter the quantity you would like to receive in the quantity box. The order limits are next to the quantity boxes. You can request several items in one order.
• Please double check your shipping address and make sure you have a complete street address. Orders cannot be shipped to PO Boxes. Orders with incomplete addresses and orders with a PO Box will not be processed.
• Multiple orders for the same publication to the same address will be canceled. Only the first order will be processed.
• All orders require 4 to 6 weeks for delivery. Rush orders are not available.
• If you need more than the maximum quantity allowed for a publication, please send an e-mail to IZlearn@cdc.gov stating the quantity needed, the purpose for which they will be used (such as an event, like a health fair), and your contact information.
• International orders cannot be placed online at this time. Please call 1-800-CDC-INFO stating the quantity needed, the purpose for which they will be used (such as an event, like a health fair), and your contact information.
