Terms of Use
- Limited quantities of the following publications are available free of charge. You may order more later.
- Please enter the number of copies you would like to receive for each item. The request limit is next to each box.
- Please double check your address! Orders are not sent if address is incorrect.
- Multiple orders for the same publication to the same address will be canceled. Only the first order will be processed.
- All orders require 4 to 6 weeks for delivery. Rush orders are not available.
- If you need more than the maximum quantity allowed for a publication, please call 1-800-CDC-INFO stating the quantity needed, the purpose for which they will be used (such as an event, like a health fair), and your contact information.
- International orders cannot be placed online at this time. Please call 1-800-CDC-INFO stating the quantity needed, the purpose for which they will be used (such as an event, like a health fair), and your contact information.
