Terms of Use

  • Limited quantities of the following publications are available free of charge.
  • Please enter the number of copies you would like to receive for each item. The request limit is next to each box.
  • Please double check your address! Orders are not sent if address is incorrect.
  • Multiple orders for the same publication to the same address will be canceled. Only the first order will be processed.
  • All orders require 4 to 6 weeks for delivery. Rush orders are not available.
  • If you need more than the maximum quantity allowed for a publication, please send an e-mail to DSTDPMatReq@cdc.gov stating the quantity needed, the purpose for which they will be used (such as an event,like a health fair), and your contact information.
