Publications Order Form

Terms of Use

• Limited quantities of the following publications are available free of charge.
• Please enter the number of copies you would like to receive for each item. The request limit is next to each box.
• Please double check your address! Orders are not sent if address is incorrect.
• Multiple orders for the same publication to the same address will be canceled. Only the first order will be processed.
• All orders require 4 to 6 weeks for delivery. Rush orders are not available.
• If you need more than the maximum quantity allowed for a publication, please send an e-mail to the Division of STD Prevention stating the quantity needed, the purpose for which they will be used (such as an event,like a health fair), and your contact information.
• International orders cannot be placed online at this time. Please send an email to the Division of STD Prevention stating the item number(s) and quantities needed. International orders are limited to 10 copies per order. Please allow 8 weeks for delivery.
