Terms of Use

  • Limited quantities of the following publications are available free of charge. You may order more later.
  • Please enter the number of copies you would like to receive for each item. The limit is next to each box.
  • If you need more than the maximum quantity allowed for a publication, please send an e-mail to dvhwi@cdc.gov stating the quantity needed, the purpose for which they will be used (such as an event, like a health fair), and your contact information.
  • International orders cannot be placed online at this time. Please send an email to dvhwi@cdc.gov stating the item number(s) and quantities needed. International orders are limited to 10 copies per order. Please allow 8 weeks for delivery.
  • Multiple orders for the same publication to the same address will be canceled. Only the first order will be processed.
  • Please double check your address! Orders are not sent if address is incorrect.
  • All orders require 4 to 6 weeks for delivery. Rush orders are not available.
