Provider Request New TA

The Provider must have CDC approval in order to submit a reactive request.  

A provider can only request Technical Assistance (TA) on behalf of a CDC Directly-Funded organization.

Locate the Request New Technical Assistance (TA) box from the dashboard.

 

 

  1. In the Request New TA box, click the Add button to add a new reactive request.
  2. The Request for a New Reactive TA page displays.

 

 

Note: A request number is generated; make note of this number.

  1. To view the New Request Details, click the plus (+) sign.
  2. The New Request Details section displays.

 

 

  1. To complete the request, answer the required questions (noted with double asterisk**).
  2. Select a subtrack topic for which you would like technical assistance (TA).
    1. Hover over any of the buttons for additional information on a subtrack.
    2. Only one subtrack can be selected per request.
    3. Once a subtrack is selected it changes colors and will display below the buttons.

 

 

  1. Answer the remaining required questions (noted with double asterisks **).
  2. Identify the organization(s) that will receive the technical assistance (TA).

Note: The organization field is blank.

 

 

  1. Click the mouse pointer in the organization field.
  2. The Provide the information for whom this request is being entered dialog box displays.

 

 

  1. Select an organization from the on behalf of drop-down list.
  2. Select a name from the Assign Contact drop-down list.
  3. Select an announcement number from the What is the Program Announcement number drop-down list.

Note: If the organization has only one announcement number, it will auto populate once selected as the on behalf of organization.

  1. Select an answer for the question, “Will you designate another organization user as the Point of Contact (POC) for this request?”.
    1. If Yes is selected, the Change Request Contact message displays as seen in the image below.
    2. If No is selected, the on behalf of organization will remain the POC. 

 

 

  1. Select an answer for the question,"Will organization be a recipient of the request?"
    1. If Yes is selected, the on behalf of organization will display in the organization field as a recipient.
    2. If No is selected, a recipient organization must be added. 
  2. In the image above, the on behalf of organization will not be a recipient on this request. Click the Save button.
  3. The application takes you back to the organization field.Click the mouse pointer in the organization field and a list of organizations will display.
    1. You can type in the name(s) of the recipient organization or you can select them from the list.
    2. Click outside of the organization field to close the list.
  4. To assign a different Point of Contact (POC), click the Change Request Contact button. 
  5. The Change Request Contact dialog box displays.

 

 

  1. Select a name from the Organization Name drop-down list.
  2. Select a name from the Request Point of Contact drop-down list.
  3. Click the Save button.

Note: The Add New Organization button does not display until the first recipient organization has been added to the request.

  1. To add a new organization, click the Add New Organization button.

Note: Before adding a new organization, check the list of existing organizations to confirm the organization you are about to add does not already exist in CTS.

  1. The Add New Organization dialog box displays.

 

 

  1. Answer the required questions (noted with double asterisks **).
  2. Click the Save button.
  3. The organization displays in the recipient organization section as seen in the image below.

 

 

  1. Click the Assign Contacts link.
  2. The Assign Contact dialog box displays.

 

 

  1. Click the Add New Contact button.
  2. The Add a New Contact dialog box displays.

 

 

  1. Enter the required information (noted with double asterisks **)
  2. Click the Save button.
  3. The Assign Contacts dialog box displays.

 

 

  1. The new contact name will automatically be selected. Click the Save button.
  2. The contact is added to the organization and can be seen in the Recipient Contact(s) column.

 

 

  1. Click in the date field to select a date from the calendar or enter a date.
    1. The start date for the request should be 15 business days after the date the request has been submitted in CTS.

 

 

  1. Select an option from the drop-down list to the question, “How would you like the TA to be delivered?”.

 

 

Note: The request can be saved even if the information provided for this page is incomplete. You can return later and complete the request.

  1. Click the Submit button.
  2. The Success message displays.

 

 

 

  1. Click the OK button.  The box closes and the dashboard displays.
