Welcome to the MERS patient under investigation (PUI) short form electronic entry portal.  This site is intended for use by State Health Departments only.  Please login using the name of your state and the password provided by the CDC MERS domestic team.

This portal will allow you to enter MERS PUI short forms electronically, print out PUI short forms you’ve already entered, and download reports of all the PUI short forms entered for your state.
