|Effective March 10, 2014, CDC's Division of Global Migration and Quarantine (DGMQ) changed its method for receiving maritime reports of non-gastrointestinal illnesses, deaths, and cumulative influenza/influenza-like illnesses. Cruise ships should report using new fillable PDF forms at  http://www.cdc.gov/quarantine/cruise-reporting-guidance.html .
As of June 10, 2014, DGMQ will no longer accept death and illness reports via the Maritime Illness and Death Reporting System (MIDRS) Website or MIDRS email. If you have any questions, please email MaritimeAdmin@cdc.gov.
