The WISEWOMAN Data Management System (DMS) was designed specifically for grantees to enter, submit and validate data for the WISEWOMAN program.
The WISEWOMAN program is administered through CDC's Division for Heart Disease
and Stroke Prevention (DHDSP). The WISEWOMAN program provides low-income, under-insured or
uninsured women with chronic disease risk factor screening, lifestyle programs, and referral
services in an effort to prevent cardiovascular disease. The priority age group is women aged
35–64 years.
The WISEWOMAN (Well-Integrated Screening and Evaluation for WOMen Across the Nation) program is located at the Centers for Disease Control and Prevention (CDC) in Atlanta, Georgia, within the Division for Heart Disease and Stroke Prevention (DHDSP). WISEWOMAN provides screening for heart disease and stroke risk factors and lifestyle programs for many low-income, uninsured, or under-insured women aged 35–64 years.
Public reporting burden of this collection of information is estimated to average 24 hours per program including the time for reviewing instructions, searching existing data sources, gathering and maintaining the data needed, and completing and reviewing the collection of information. An agency may not conduct or sponsor, and a person is not required to respond to a collection of information unless it displays a currently valid OMB control number.
