Site: Potable Water-Engine Control Room
Violation: Legionella sampling of technical water sources and hot water return lines were not tested as outlined in the VSP 2025 Environmental Public Health Standards.
Recommendation: Sample the potable water system for Legionella at least every 6 months. Ensure sample sites are representative of the potable water and RWF systems and number of samples depends on the size and complexity of the vessel. Take follow-up actions when Legionella is detected based on the type of Legionella found, the concentration (for example in CFU/ml), and the extent of colonization. Actions can include (1) Review of the water management program; (2) Root cause analysis to understand possible reasons for Legionella growth; (3) Updates to the water management program if needed. Consider retesting fixtures or devices where Legionella was detected to confirm the effectiveness of the response and to determine if remediation is needed. For additional guidance, see ASHRAE Guideline 12, Appendix C5. The Legionella sampling plan must include a selection of the following locations: (1) Potable water storage tanks; (2) Water heaters and hot water storage; (3) Hot water return lines; (4) Representative points of use in cabins, galleys, and recreational areas. Include a mix of fixture types (showers, sink faucets, tubs). Include potable water samples when temperatures are in the range favorable for Legionella growth (77?113F, 25?45C); (5) Decorative water features; (6) Misting systems; (7) Salon and pedicure devices; (8) Technical water sources with a risk of exposure via aerosolization to crew or passengers, such as deck washing.

Site: Potable Water-Deck 3 - Potable Water Hose Locker
Violation: The 'Potable Water Only' identifier on 3 potable water hose lines were faded and hard to read.
Recommendation: Label potable water hose lockers ?POTABLE WATER HOSE AND FITTING STORAGE? in letters at least 13 millimeters (0.5 inch) high.

Site: Potable Water-Engine Control Room - Potable Water Bunkering Log
Violation: The potable water bunkering log for 9/29/2025 had recorded time inaccuracies. Start time for potable water tank 2213 had a recorded start time of 2:30 pm, however, the recorded hourly halogen and pH test monitoring was logged as 6:00 am - 10:00 am. The final entry was recorded at 3:30 pm. As per engine room crew, potable water production data entry was inadvertently recorded on the potable water bunkering log. Additionally, a potable water bunkering log entry on 9/5/2025 for potable water tank 2113 did not have the time recorded for the halogen and PH test.
Recommendation: Maintain accurate records of bunkering and production halogentation and pH control monitoring on the vessel for 12 months and ensure these records are available for review during inspections.

Site: Medical-Acute Gastroenteritis (AGE) Log
Violation: A crew member with AGE symptoms did not report to medical within a reasonable time frame. The crew member was a food handler that reported to medical on 10/30/2025 at 11:00 am and reported illness onset of 10/30/2025 at 3:00 am. The crew member had reported to work with the AGE symptoms prior to reporting to medical. Corrective actions were recorded outlining the steps taken in the galley to remediate the issue.
Recommendation: Ensure food employees who have conditions or symptoms of boils, open sores, infected wounds, diarrhea, jaundice, fever, vomiting, sore throat with fever, or discharges from the nose or mouth report these conditions or symptoms to the vessel's medical staff and are restricted from working with exposed food, warewashing, clean equipment, utensils, linens, and unwrapped single-service and single-use articles.

Violation: Water dripped from the back right deckhead seam onto boxes of food below.
Recommendation: Protect food from contamination by storing the food: (1) Covered or otherwise protected; (2) In a clean, dry location; (3) Where it is not exposed to splash, dust, or other contamination; (4) At least 150 millimeters (6 inches) above the deck; (5) At least 127 millimeters (5 inches) for food in packages and working containers on pallets, skids, and racks; (6) At least 150 millimeters (6 inches) above the deck on shelving units.

Violation: The coffee machine had slotted fasteners on the filter where the port-a-filter connects to the coffee machine.
Recommendation: Prohibit the use of exposed slotted screws, Phillips head screws, or pop rivets in these areas.

Violation: A half inch of water was inside the deckhead light fixture. This was corrected immediately.

Violation: An open gap was to the right of the undercounter refrigerator where it met the bulkhead. This was corrected immediately.

Violation: A wet paper towel and a small amount of food debris was beneath the cleaning materials locker. This was cleaned immediately.

Recommendation: Ensure the light intensity is at least 220 lux (20 foot-candles) at the dispensing nozzle(s) and the filling surface(s) during cleaning operations of bulkhead-recessed, bulkhead-mounted, freestanding potable water filling stations and drinking fountains outside of food areas for crew and passengers.

Site: Integrated Pest Management-Rat Guards - Ship Aft
Violation: Two of the rat guards were ineffectively installed on the aft mooring lines.
Recommendation: Protect entry points where pests may enter the food areas. Ensure rat guards are made of a durable and nonchewable material. Use rat guards according to the manufacturer?s specifications for the effective prevention of pest migration. Ensure rat guards are on all lines that go ashore on arrival and until 1 hour before the ship leaves port. Ensure each line has at least one rat guard placed either as far as practicable from the pier or as far as practicable from the ship but still close enough to stuff any openings with nonchewable material. Ensure rat guards are not staggered on adjacent lines, and a group of lines do not merge onto one rat guard.
