Site: Buffet-Terrace Buffet - Aft
Violation: There was one live housefly observed in the buffet service area near the bagels and other bread items.
Recommendation: Effectively control the presence of insects, rodents, and other pests to minimize their presence in the food storage, preparation, and service areas and warewashing and utensil storage areas aboard a vessel.

Site: Provisions-Dairy Refrigerator
Violation: A cardboard box containing two wheels of packaged bleu cheese was leaking. The inside of the cardboard box was lightly saturated with an oily substance and the metal shelving had some oily debris on it. The cheese was promptly removed.
Recommendation: Ensure food packages are in good condition and protect the integrity of the contents so that the food is not exposed to adulteration or potential contaminants. Ensure canned goods with dents on end or side seams are not used.

Site: Buffet-Waves Grill
Violation: There was a large, opened container of mayonnaise stored in the reach in refrigerator which was not labeled for 7-day discard. Staff removed this item immediately.
Recommendation: Ensure refrigerated, ready-to-eat, potentially hazardous food: (1) Prepared on a vessel and held refrigerated for more than 24 hours is clearly marked at the time of preparation to indicate the date or day by which the food must be consumed (7 calendar days or fewer from the day the food is prepared). The day of preparation is counted as day 1. (2) Prepared and packaged by a food-processing plant and held on the vessel after opening for more than 24 hours must be clearly marked at the time the original container is opened to indicate the date by which the food must be consumed (7 calendar days or fewer after the original container is opened). The day of opening is counted as day 1.

Site: Potable Water-Cross Connection Control Program Listing
Violation: Two backflow preventers were found on shower hoses in the medical center but they were not listed on the list of backflow prevention devices that were visually inspected at least annually. This was corrected during the inspection.
Recommendation: Ensure the cross-connection control program includes at a minimum: a complete listing of cross-connections and the backflow prevention method or device for each, so there is a match to the plumbing system component and location. Include air gaps on the listing. Ensure a schedule for inspection frequency is set. Maintain a log documenting the inspection and maintenance in written or electronic form, and ensure it is available for review during inspections.

Site: Potable Water-Engine Room - Evaporator 2
Violation: The vent of the reduced pressure assembly was blocked so that water could not vent out of the device. A metal plate had been screwed onto the bottom of the vent of the device. The plate was immediately removed and water that had collected in the vent poured out.
Recommendation: Maintain backflow prevention devices in good repair. Do not block the vents of backflow prevention devices.

Site: Integrated Pest Management-Mooring Lines
Violation: At least 5 mooring lines were not adequately protected from pest entry. The rat guard did not completely circle the mooring lines.
Recommendation: Protect entry points where pests may enter the food areas.

Violation: The drain of the handwash sink was leaking onto the deck tiles below.
Recommendation: Ensure black and gray water is discharged to the vessel's wastewater disposal system and does not pool on the deck.

Violation: At the connection with the bottom of the sink, the top of the drain pipe was soiled with excessive debris.
Recommendation: Ensure nonfood-contact surfaces of equipment are kept free of an accumulation of dust, dirt, food residue, and other debris.

Violation: The backflow preventer for the ice machine was soiled with debris which was flaking off the backflow preventer.The debris appeared to be hard water deposits. Staff corrected this by replacing the device.
Recommendation: Maintain backflow prevention devices in good repair.
