During the last voyage, a passenger had diarrhea and an underlying illness. Per the doctor, this individual did not have diarrhea associated with an infectious process. However, this individual was dispensed anti-diarrheal medication but was not listed in the standardized gastrointestinal illness log.

Recommendation:
The log shall list:
(1) All reportable cases of gastrointestinal illness; (2) All passengers and crew members who are dispensed antidiarrheal medication from the master of the vessel, the medical staff, or other designated staff.

A review of the potable water production log noted more than 1/2 hour and up to 1 hour between the start of production and the first test showing a free chlorine residual of 2 ppm or greater. Per the engineers, the start time in the log was the start time for the evaporators and not the start time of potable water production. According to the engineers, the water produced was dumped overboard until the salinity levels and pressure were stable.

Recommendation:
Ensure that the start time in the potable water production log is the start of potable water production and not the start time for the evaporators.

The comprehensive cross-connection control program did not list all of the connections to the potable water system. The small lines for filling the acid and chlorine tanks in the engine room were not included. The new "Q Age" tub in the spa was not included. These connections were adequately protected.
A review of the program showed that the devices listed were not always correct in both the manufacturer's model number designation or in the quantity of devices used. This was corrected for the connections reviewed.

Recommendation:
Ensure that the cross-connection control program includes at minimum: a complete listing of cross-connections and the backflow prevention method/device for each, so that there is a match to the plumbing system component and location, and an inspection frequency. Ensure that air-gaps are included in the listing.

The potable water line supplying the hose connection above the chlorine tank for distilled water was not fitted with a backflow prevention device. There was an air gap between this connection and the chlorine tank below. There was a hose connected to a connection nearby that was fitted with a backflow prevention device. This hose was very long and could reach all the tanks. Per the engineers the hose connection that was not protected was not used as a hose connection.

Recommendation:
Remove the hose connection fitting or install the appropriate backflow prevention device.

Recommendation:
Ensure that anti-entrapment drain covers or other drains that prevent entrapment hazards as specified in U.S. Consumer Product Safety Publication 363-009801 (dual drains/channel drains) are provided on swimming pools and whirlpool spas. Ensure that drain covers have either the plumbing/engineering approving organization stamp and flow rate affixed to the cover, or that a letter is maintained aboard the vessel certifying that the cover meets the safety requirements outlined in AMSE/ANSI A112.19.8M or an equivalent standard.

This may be accomplished by methods such as locating paper towel dispensers at sinks and waste containers near the room door, installing mechanically operated doors, door removal, or other effective means.
