Item No.: 08
Site: Potable Water-
Violation: The potable water connection to the ice machine in the crew mess was not fitted with a backflow prevention device. There was no backflow prevention device for the urinal in the men's toilet room in the spa.
Recommendation: Ensure that backflow preventers are installed when air gaps are impractical or when water under pressure is required.

Site: Potable Water-
Violation: The records for the potable water tanks did not show that the tanks were flushed to a free residual halogen level of less than or equal 5 ppm before putting the tanks back in to service after disinfection.
Recommendation: Ensure that documentation of any inspection, cleaning, disinfection (including concentration and contact time of disinfectant), and flushing is maintained for 12 months and is available to the VSP for review during inspections. Ensure that the disinfected parts of the system are flushed with potable water until the free residual halogen is at or below 5.00 mg/L (ppm).

Site: Potable Water-Pool and Whirlpool Machinery Rooms
Violation: There were small diameter lines that had threaded hose connections where the potable water lines were not fitted with backflow prevention devices. This was noted in the lido whirlpool, pavilion whirlpool and hydropool rooms. There was a flexible hose connected over the pavilion whirlpool acid tank that could reach in to the tank and a backflow prevention device was not installed on the potable water line.
Recommendation: Ensure that backflow preventers are installed when air gaps are impractical or when water under pressure is required.

Site: DO NOT USE (whirlpool/spa)-
Violation: The inspection and sedimentation test records for the sand filters for the month of December were not clear. The AMOS system showed the inspections for January but according to the staff the inspections and tests were performed in December. There was no indication in the history of this nor was there any record of the result of the inspections and tests.
Recommendation: Ensure that a written or electronic record of whirlpool spa filter inspection, backwashing, core sample sedimentation test, and shock halogenation (concentration and contact time) is maintained and available for review during inspections.

Site: Housekeeping-Deck 9 Lido Toilet Rooms
Violation: There was no means to exit the toilet rooms without the use of bare hands.
Recommendation: Equip public toilet facilities so that persons exiting the toilet room are not required to handle the door with bare hands. This may be accomplished by methods such as locating paper towel dispensers at sinks and waste containers near the room door, installing mechanically operated doors, door removal, or other effective means.

Site: Medical-
Violation: There was only an initial interview with cabin mates and close contacts of crew members with gastrointestinal illness. There were no 24-hour or 48-hour interviews conducted. This was noted during the 12/11/08-01/02/09 cruise.
Recommendation: Ensure that medical staff conduct verbal interviews with asymptomatic cabin mates or immediate contacts to confirm their condition, advise them of the hygiene and handwashing facts, and instruct them to report immediately to medical if they develop illness symptoms. Ensure that medical staff conduct daily verbal interviews with asymptomatic crew until 48 hours after onset of the ill crew members symptoms.

Site: Potable Water-
Violation: There were a number of connections to the potable water system that were not listed in the cross-connection control program. The connections in the crew galley, mess, potwash and scullery to detergent dispensers, threaded connections at bucket fill stations and warewash machines were not listed. The potable water connections to the toilets and showers in the women's and men's changing areas in the spa were not listed.
Recommendation: Ensure that the cross-connection control program includes at minimum: a complete listing of cross-connections and the backflow prevention method/device for each, so that there is a match to the plumbing system component and location, and an inspection frequency. Ensure that air-gaps are included in the listing.

Site: Buffet-Lido Port Side
Violation: There were two unlabeled working containers of cooking oil and one unlabeled working container of salt.
Recommendation: Ensure that working containers holding food or food ingredients that are removed from their original packages for use on the vessel, such as cooking oils, flour, herbs, potato flakes, salt, spices, and sugar are identified with the common name of the food.

Site: Galley-Queen's Grill
Violation: There were paper towels with food debris in the waste receptacle for the handwash station adjacent to the grill. This receptacle did not have a tight fitting lid.
Recommendation: Ensure that receptacles and waste-handling containers for refuse, recyclables and for use with materials containing food residue are insect- and rodent-resistant and have tight fitting lids.

Site: Galley-Queen's Grill Dishwash Area
Violation: One nozzle on the final rinse spray for the conveyor dishwash machine was clogged.
Recommendation: Ensure that all of the nozzles are functioning correctly.

Site: Potable Water-Soda Carbonators
Violation: There were no adequate backflow prevention devices on the carbonators in the Queen's grill service bar, deck 3 main galley service bar, and the Cafe Carinthia.
Recommendation: Install continuous pressure backflow prevention devices on the potable water line entering the carbonator. Ensure that the devices are not made of copper alloys. Ensure that the device is intalled such that there are no copper or brass fittings between the device and the carbonator.

Site: Galley-Britania -- Deck 3 Room Service Area
Violation: There drip tray housing on the previously cleaned flat grill was soiled.
Recommendation: Ensure that nonfood-contact surfaces of equipment are kept free of an accumulation of dust, dirt, food residue, and other debris.

Site: Galley-Britania -- Deck 2 Beverage Station
Violation: The two bulk milk dispensing tubes were longer than one inch.
Recommendation: Ensure that the bulk milk container dispensing tube is cut on the diagonal leaving no more than 25mm (1 inch) protruding from the chilled dispensing head.

Site: Galley-Britania -- Deck 2 Dishwash Area
Violation: The final rinse temperature on the conveyor dishwash machine was measured between 120F and 130F. The dishwash machine was in use. This was corrected.
Recommendation: In a mechanical operation, ensure that the temperature of the fresh hot water sanitizing rinse as it enters the manifold is not more than 90C (194F), or less than: (1) For a stationary rack, single temperature machine, 74C (165F); or (2) For all other machines, 82C (180F); or (3) A utensil surface temperature of 71C (160F) as measured by an irreversible registering temperature indicator shall be achieved. (4) The maximum temperature of 90C (194F), does not apply to the high pressure and temperature systems with wand-type, hand-held, spraying devices used for the in-place cleaning and sanitizing of equipment such as meat saws.
