Site: Potable Water-
Violation: On 12/15/07, there was no record of the manual testing for free chlorine residual in the potalble water during production. The evaporator chart recorder was not working and the evaporator was operated for 12 hours.
Recommendation: Ensure that free residual halogen monitoring is performed at least hourly during the bunkering of potable water and performed at least once every 4 hours during the onboard production of potable water.

Site: Potable Water-Cross Connection Control Program
Violation: The cross connection control program did not list the air gaps for the Ecolab chemical dispensers.
Recommendation: Ensure that the cross-connection control program includes at minimum: a complete listing of cross-connections and the backflow prevention method/device for each, so that there is a match to the plumbing system component and location, and an inspection frequency. Ensure that air-gaps are included in the listing.

Site: Potable Water-Engine Room
Violation: The vessel used the ISO system for striping the water lines. A blue stripe designated fresh water, blue stripe and green stripes designated potable water, and blue and grey stripes designated distillate water. In the engine room, the line supplying the water fountain was striped only blue. In the garbage room, the line supplying the hand wash station was also striped blue.
Recommendation: Ensure that potable water piping and fittings are painted or striped auxiliary blue, or in accordance with ISO 14726, at 5 m (15 feet) intervals on each side of partitions, decks, and bulkheads in all spaces, except where the decor would be marred by such markings.

Site: DO NOT USE (whirlpool/spa)-Mid Port Jacuzzi
Violation: On 1/14, 1/16, 1/25, and 1/28/07, there was no notation on the records that the jacuzzi was super-halogenated. Per staff, super-halogenation was conducted nightly.
Recommendation: Ensure that a written or electronic record of whirlpool spa filter inspection, backwashing, core sample sedimentation test, and shock halogenation (concentration and contact time) is maintained and available for review during inspections.

Violation: There was no standard safety precautions and risks sign, warning against use by particularly susceptible people, such as those who are immunocompromised.
Recommendation: Ensure that a sign is installed near the whirlpool spas that lists standard safety precautions and risks, warning against use by particularly susceptible people, such as those who are immunocompromised.

Site: Medical-
Violation: There was no documentation of the 48-hour interview with asymptomatic cabin mates of crew members ill with gastrointestinal illness. The contact was isolated for 24 hours and the initial and 24-hour interviews were documented.
Recommendation: Ensure that medical staff conduct verbal interviews with asymptomatic cabin mates or immediate contacts to confirm their condition, advise them of the hygiene and handwashing facts, and instruct them to report immediately to medical if they develop illness symptoms. Ensure that medical staff conduct daily verbal interviews with asymptomatic crew until 48 hours after onset of the ill crew members symptoms. Ensure that documentation of the interviews is maintained onboard.

Site: Galley-Deck 2 & 3 Fish Stations
Violation: The sealant used at the grease drain on the underside of the flat and grooved grills was peeling, making cleaning difficult.
Recommendation: Remove the peeling sealant and replace as necessary.

Violation: One of the electrical cords to the broiler sections of the Lang steak grill was covered with a loose material, making cleaning difficult.
Recommendation: Ensure that nonfood-contact surfaces of equipment that are exposed to splash, spillage, or other food soiling or that require frequent cleaning are constructed of a corrosion-resistant, nonabsorbent, and smooth material.

Violation: In the technical compartment at the corner of the buffet line, the back pipe had damaged insulation, making cleaning difficult.
Recommendation: Repair the damaged insulation.

Violation: There were several soiled glasses stored on the clean rack.
