Site: Medical-Collection Containers
Violation: The Para-Pak C&S specimen collection containers expired in 2006.
Recommendation: Check all of the Para-Pak speciment collection containers and ensure that expired containers are discarded. Ensure that at least 10 clinical specimen collection containers for both viral and bacteriological agents (10 each), as well as the proper shipping containers and labels for same, are maintained by the medical staff.

Violation: The distribution system halogen pumps were not wired to automatically switch over in case the primary pump failed, an increase in demand occurred, or the low chlorine alarm sounded.
Recommendation: Ensure that at least one backup halogen pump is available with automatic switchover to maintain the free residual halogen in the event that the primary pump fails, an increase in demand occurs, or the low chlorine alarm sounds.

Site: Galley-Deck 6 Main Galley Pastry Dispensing
Violation: Open cell foam insulation was found around refrigeration unit #5 technical space access panel. The insulation was saturated with water. The water was seeping from the insulation and down the front of the refrigeration unit.
Recommendation: Ensure that nonfood-contact surfaces of equipment that are exposed to splash, spillage, or other food soiling or that require frequent cleaning are constructed of a corrosion-resistant, nonabsorbent, and smooth material.

Site: Galley-Starboard Diswash Unit
Violation: The starboard side flight-type dishwash unit sanitizing rinse was measured at 190F at the spray nozzle. The external temperature gauge indicated 176F.
Recommendation: Ensure that warewashing equipment is maintained in good repair and proper adjustment including: (1) Being maintained in a state of repair and condition that meets the standards of the materials, design, and construction of these guidelines; and (2) Maintaining water pressure, and water temperature measuring devices in good repair and ensuring their accuracy within the intended range of use.

Recommendation: Ensure that the vessel's Integrated Pest Management Plan is evaluated for effectiveness every 2 years or whenever there is a significant change in the vessel's structure such as a renovation or operation.

Violation: The crew toilet outside the whirlpool machinery room on deck 6 did not have a means of egress without touching the handle with their bare hands.
Recommendation: Equip public toilet facilities so that persons exiting the toilet room are not required to handle the door with bare hands. This may be accomplished by methods such as locating paper towel dispensers at sinks and waste containers near the room door, installing mechanically operated doors, door removal, or other effective means.

Violation: There was no readily accessible handwashing station at the men's spa toilet area. A common area door to the changing room must be accessed in order to reach the handwashing station.
