Violation: THE NON STICK RESIN FINISH WAS SCRATCHED, SCORED AND CORRODED ON NUMEROUS BAKING RINGS, MAKING THEM DIFFICULT TO CLEAN.
Recommendation: 7.4.1.1.2 Materials that are used in the construction of multiuse utensils and food-contact surfaces of equipment shall be: (1) Durable, corrosion-resistant, and nonabsorbent; (2) Sufficient in weight and thickness to withstand repeated warewashing; (3) Finished to have a smooth, easily cleanable surface; and (4) Resistant to pitting, chipping, crazing, scratching, scoring, distortion, and decomposition.

Violation: A COOK REMOVED A PAN FROM THE DIRTY SIDE OF THE POTWASH AREA TO USE TO STRAIN PASTA. HE WAS INFORMED THE DISH WAS SOILED AND HE RETURNED AND OBTAINED A COLANDER FROM THE CLEAN SIDE OF THE POTWASH AREA. HE PLACED THE COLANDER DIRECTLY ON THE DECK TO DRAIN THE PASTA. THE STAFF STOPPED THE COOK FROM USING THIS PROCEDURE TO DRAIN THE PASTA.
Recommendation: 7.2.1.2.1.12 The person in charge of the food operations on the vessel shall ensure that: (12) Employees are properly trained in food safety as it relates to their assigned duties.

Violation: CONDENSATION HAD COLLECTED ON THE DECKHEAD AND ON THE CONDENSER UNITS OF THE ICE CREAM AND FRESH FROZEN FREEZERS. SOME CONDENSATION WAS ALSO NOTED ON TOP OF BOXES OF FOOD.
Recommendation: 7.3.3.5.2 Food may not be stored: (1) In locker rooms; (2) In toilet rooms; (3) In dressing rooms; (4) In garbage rooms; (5) In mechanical rooms; (6) Under sewer lines that are not continuously sleeve welded; (7) Under leaking water lines, including leaking automatic fire sprinkler heads, or under lines on which water has condensed; (8) Under open stairwells; or (9) Under other sources of contamination from nonfood items such as ice blocks, ice carvings and flowers.

Violation: THE SICK CREW MEMBER POSITION/JOB WAS NOT WRITTEN ON THE GASTROINTESTINAL ILLNESS SURVEILLANCE LOG.
Recommendation: 4.1.2.1.4 The gastrointestinal illness surveillance log entry for each passenger or crew member shall contain the following information: (1) The first date of clinic visit or report to staff of illness; (2) The person's name, age and gender; (3) A designation as passenger or crew member; (4) Crew member position or job on the vessel, if applicable; (5) Cabin number; (6) Meal seating information; (7) Date and time of illness onset; (8) Illness symptoms, including the presence of the following selected signs and symptoms: numbers of episodes each of diarrhea and vomiting per day, bloody stools, fever, recorded temperature; (9) Notation on whether or not a stool specimen was requested and received; (10) Use of antidiarrheal medication; and (11) The presence of underlying medical conditions which may affect interpretation of acute gastrointestinal illness for example diabetic diarrhea, inflammatory bowel disease, gastrectomy or others.

Recommendation: 7.4.2.2.2 CIP equipment shall meet the following criteria: (1) It shall be designed and constructed so that cleaning and sanitizing solutions circulate throughout a fixed system and contact all interior food-contact surfaces, and the system is self-draining or capable of being completely drained of cleaning and sanitizing solutions; or (2) CIP equipment that is not designed to be disassembled for cleaning shall be designed with inspection access points to ensure that all interior food-contact surfaces throughout the fixed system are being effectively cleaned.

Recommendation: 7.6.2.1.2 Poisonous or toxic materials used in food area cleaning and maintenance shall be stored so they cannot contaminate food, equipment, utensils, linens, and single-service and single-use articles by separating the poisonous or toxic materials by storing in a cleaning materials locker.

Recommendation: 7.3.4.2.2 If raw, raw-marinated, partially cooked, or marinated-partially cooked fish are served in ready-to-eat form: (1) The person in charge shall record the freezing temperature and time to which the fish are subjected and shall retain the records on the vessel for 90 calendar days beyond the time of service or sale of the fish; or (2) If the fish are frozen by a supplier, a written agreement or statement from the supplier stipulating that the fish supplied are frozen to a safe temperature and a time.

Violation: THE TOILET SEAT IN THE CHILD TOILET ROOM DOES NOT APPEAR TO BE CHILD-SIZED.
Recommendation: 10.2.1.1.1 Child-size toilet and handwashing facilities shall be provided, if toilet rooms are located in a child-activity center. Verify that the seat is child-sized or install such a seat.

Violation: THE CHILDREN'S TOILET WAS NOT SUPPLIED WITH DISPOSABLE GLOVES OR SANITARY WIPES. THERE WAS NO SIGN POSTED IN THE CHILDREN'S TOILET ADVISING CHILD CENTER STAFF TO WASH THEIR HANDS AND THE CHILDREN'S HANDS AFTER ASSISTING CHILDREN TO USE THE TOILET.
Recommendation: 10.2.1.1.2 Each toilet facility shall be provided with a supply of toilet tissue, disposable gloves, and sanitary wipes. 10.2.1.1.5 Signs shall be posted in children's toilet room advising the providers to wash their hands and the children's hands after assisting children use the toilet.

Violation: THERE WERE SEVERAL 4 HOUR PERIODS WHERE THE SHIP WAS PRODUCING POTABLE WATER AND THE FREE CHLORINE RESIDUAL WAS BELOW 2.0 PPM. MANUAL TESTS WERE MADE OF THE RESIDUALS WITH REMARKS OF ADJUSTMENT, BUT THERE WAS NO REPEAT TEST RECORDED TO SHOW THE RESIDUAL WAS EVER CORRECTED TO A MINIMUM OF 2 PPM.
Recommendation: 5.2.1.1.1 Potable water shall be continuously halogenated to at least 2.0 mg/L (ppm) free residual halogen at the time of bunkering or production with an automatic halogenation device.

Violation: THE REDUCED PRESSURE BACKFLOW PREVENTION DEVICES USED THROUGHOUT THE SHIP HAVE NOT BEEN TESTED IN-PLACE.
Recommendation: 5.7.2.2.2 Backflow prevention devices requiring testing, for example reduced pressure backflow preventer and double check valves with test cocks, shall be inspected and tested with a test kit at least annually. Test results showing the pressure differences on both sides of the valves shall be maintained for each device.
