DURING THE MOST RECENT CRUISES, THE MEDICAL STAFF WAS USING THE SPECIAL REPORT TO SUBMIT ROUTINE REPORTS. WHEN ASKED ABOUT THIS PRACTICE, THE STAFF INDICATED THIS WAS THE ONLY WAY TO SUBMIT GI REPORTS AFTER THE SUBMISSION OF THE 4-HOUR REPORT. THE MEDICAL STAFF WAS SUBMITTING THE 4-HOURS REPORT AT APPROXIMATELY 0200 (BASED ON AN EXPECTED ARRIVAL OF 0600). THE MEDICAL STAFF WAS ALSO SUBMITTING ROUTINE REPORTS BETWEEN TWO US PORTS, WHICH IS NOT REQUIRED BY THE OPERATIONS MANUAL.

Recommendation:
Ensure that the master, the medical staff, or other designated staff of a vessel destined for a U.S. port from a foreign port submits at least one standardized gastrointestinal illness report based on the number of reportable cases in the gastrointestinal illness log to the VSP no less than 24 hours, but not more than 30 hours before the vessel's expected arrival at the U.S. port. If the number of cases changes after submission of the initial report, ensure that an updated report is submitted no less than 4 hours before the vessel's arrival at the U.S. port.

Recommendation:
Ensure that all flexible shower hoses are fitted with appropriate backflow protection devices. It may be necessary to increase the frequency that these devices are inspected as part of the comprehensive cross-connection control plan to make sure they are not removed and lost.

THERE WAS A DISCONNECTED 'JUMPER LINE' LINE ON THE BACKWASH DISCHARGE LINE FROM THE MINERALIZERS. IT WAS STATED THAT THE LINE DRAINED DIRECTLY INTO THE GRAY WATER SYSTEM. WHILE AT THE TIME OF THE INSPECTION THE LINE WAS DISCONNECTED THE USE OF JUMPER LINES WITHOUT APPROPRIATE BACKFLOW PROTECTION INCREASE THE RISK OF ACCIDENTAL CROSS-CONNECTIONS.
