Site: Galley-Galley 14
Violation: The technical space inside the firehose cabinet in the galley opposite the potwash had over 2 inches of standing water on the deck inside creating a place for pest harborage.
Recommendation: Inspect technical areas in galleys to prevent buildup of water and food debris that could attract pests.

Violation: Over one inch of water was standing inside the cabinet on the deck by the entrance to this galley. The standing water was potentially harborage for pests in the adjacent galley.

Violation: The backflow preventer on the counter sink for the bain marie across from the flat grill was constantly leaking onto the countertop and not functioning properly. The area was not open.

Site: Potable Water-System Maintenance Records
Violation: The hot and cold water lines from passenger cabins E403 to E407 were replaced on 5 July, but the final chlorine value after the pipe disinfection was not documented before the pipe was placed into service.
Recommendation: Flush the disinfected parts of the system with potable water or otherwise dechlorinate until the free residual halogen is less than or equal to 5.00 mg/L (ppm). Document the free halogen test result.

Site: Recreational Water Facilities-Neptune Pool Life Saving Equipment
Violation: The top of the shepherd's hook for the Neptune Pool was tied to a staircase railing which did not allow for easy access in case of an emergency. The rope for the life ring flotation device for the Neptune Pool was also tied to a railing which did not allow for easy access in case of an emergency.
Recommendation: Provide a rescue or shepherd?s hook and an approved flotation device at a prominent location (visible from the full perimeter of the pool) at each RWF that has a depth of 1 meter (3 feet) or greater. Mount these devices in a manner that allows for easy access during an emergency. Ensure the pole of the rescue or shepherd?s hook is long enough to reach the center of the deepest portion of the pool from the side plus 2 feet. Ensure it is a light, strong, nontelescoping material with rounded, nonsharp ends. Ensure the approved flotation device includes an attached rope that is at least 2/3 of the maximum pool width.

Violation: Three large boxes of opened cereal and two glass serving jars containing food were uncovered and unprotected inside this room. It was after 1200 and lunch was in service.
Recommendation: Protect food from contamination by storing the food: (1) Covered or otherwise protected; (2) In a clean, dry location; (3) Where it is not exposed to splash, dust, or other contamination; and (4) At least 15 centimeters (6 inches) above the deck.

Violation: Combination oven DEKL16607 was not operating properly and clear water was dripping from a panel under the machine onto the deck.
Recommendation: Ensure nonfood contact equipment is maintained in good repair and proper adjustment including: (1) Equipment maintained in a state of repair and condition that meets the materials, design, construction, and operation specifications of these guidelines; (2) Equipment components such as doors, seals, hinges, fasteners, and kick plates are kept intact, tight, and adjusted in accordance with manufacturer's specifications.

Violation: The area behind the bar on the shelves where open liquor bottles are stored was constructed of wood. The wood was worn and in disrepair down to the bare particle board making this surface difficult to clean.
Recommendation: Ensure nonfood-contact surfaces are free of unnecessary ledges, projections, and crevices, and designed and constructed to allow easy cleaning and to facilitate maintenance.

Violation: The previously cleaned juice dispenser had more than a day's accumulation of orange juice on the surface of the juice containers.

Site: Provisions-Frozen Vegetable and Frozen Dairy
Violation: The bulkhead in both areas had a considerable amount of frozen condensate. The provisions in both freezers also had evidence of frozen condensate drips on the surfaces of the boxes. The condenser units in both rooms had a significant buildup of ice in the back of the units. Engineering explained this may be due to a malfunction during the defrost cycle.
Recommendation: Ensure decks, bulkheads, deckheads, and attached equipment in food preparation, warewashing, pantries, and storage areas are cleaned as often as necessary.

Violation: The return ventilation ducts directly above the service bar were heavily soiled with dust and debris beyond a day's accumulation.
Recommendation: Ensure decks, bulkheads, deckheads, and attached equipment in food preparation, warewashing, pantries, and storage areas are cleaned as often as necessary.

Violation: Water was dripping from a deckhead panel onto the deck in front of the counter beside under counter cooler #8. The area was not open for service.

Violation: The deckhead over the employee side of the buffet line had two deckhead panels that had become unfastened and were sagging down exposing a dirty technical space making the area hard to clean. The area was open for lunch but no food was impacted.
Recommendation: Ensure decks, bulkheads, and deckheads in food preparation, warewashing, pantries, and storage areas are maintained in good repair.

Violation: The deck in front of the mobile trolley on both port and starboard sides of the ship containing pitchers of water was stained with food debris. The deck in this area was porous and difficult to clean.

Violation: The undercounter dishwasher in this area was resting on a 1 inch by 2 inch piece of wood directly on the deck. The wood was soiled and wet. The area was open for lunch.

Ensure surfaces subject to routine splashes, spillage, or other soiling during normal use have easily cleanable features.

Violation: The average light intensities on two passenger buffet lines during lunch service was below the required 220 lux on the 2 meter serving lines. The two lines noted were the starboard line, serving custard and cakes and the forward starboard line serving rice and beef.
Recommendation: Ensure the light intensity is at least 220 lux (20 foot candles) on food preparation surfaces, and at a distance of 75 centimeters (30 inches) above the deck in food preparation areas, handwashing facilities, warewashing areas, equipment, and utensil storage, pantries, toilet rooms, and consumer self-service areas.
