The AGE log for the 4-26 April voyage contained no temperatures recorded for 10 cases. Staff stated that some of them were passengers reporting from their cabins and refusing consultation but others like a crew member (nurse) was an oversight. In addition, the underlying illness column had no information for 7 cases.

Recommendation:
Ensure the AGE surveillance log entry for each passenger or crew member contain the following information in separate columns: (d) recorded temperature; (16) presence of underlying medical conditions that may affect interpretation of AGE.

Violation:
The review of the AGE logs found that not always notes documented if the symptomatic crew members had immediate contacts or significant others, other than cabin mates.
|

Recommendation:
When any crew member (food or nonfood employee) meets the case definition for AGE, take the following actions: (1) restrict exposure to symptomatic crew member(s); (2) undergo a verbal interview with medical or supervisory staff, who will confirm their condition, provide facts and a written fact sheet about hygiene and handwashing, and instruct them to report immediately to medical if they develop illness symptoms; (3) complete a verbal interview daily with medical or supervisory staff until 48 hours after the ill crew members? symptoms began. Ensure that the first verbal interview is conducted within 8 hours from the time the ill crew member initially reported to the medical staff. If the asymptomatic immediate contact or cabin mate is at work, ensure that he or she must is contacted by medical or supervisory staff as soon as possible. Document the date and time of verbal interviews. An immediate contact means sharing a cabin either full or part time or sharing its bathroom facilities. This includes boyfriends, girlfriends, spouses, or other significant others. If the symptomatic crew member has no cabin mate or other immediate contact, this must be documented.

Violation:
The bunker hoses in the hose cabinet and the one being used the day of the inspection had very faded lettering and the 'POTABLE WATER ONLY' message could barely be read. In addition, the cruise ship hose was connected to two black hoses, a backflow preventer, and a meter installed by the port personnel. It was unclear if these connections were cleaned and flushed before use.

Recommendation:
Label potable water hoses with the words ?POTABLE WATER ONLY? in letters at least 13 millimeters (0.5 inch) high at each connecting end. Flush potable water hoses with potable water before being used and drain after each use.

The reduced pressure zone (RPz) backflow prevention device at the potable water transfer to the technical water tanks was continuously leaking from a coupling and not from the relief vent. The previous inspection of June 2016 found this RPz leaking from the relief vent.

Staff stated that they were cleaning and disinfecting the hair and lint strainers and their housings weekly. However, the record documented this procedure only for the hair and lint strainer housings and not for the strainers.

Recommendation:
Clean, rinse, and disinfect the hair and lint strainer and hair and lint strainer housing on all RWFs weekly. Ensure disinfection is accomplished with an appropriate halogen-based disinfectant. At a minimum, use a 50-ppm solution for 1 minute, or equivalent CT value. Maintain records on all inspection and cleaning procedures.

The active monitoring logs listed numerous inspections conducted by over 20 crew members that IPM training documentation could not be found. Staff stated that an electronic system that could be used to try to find if these crew members had IPM training could not be accessed.

Recommendation:
Ensure the IPM Plan includes provisions for logs for active monitoring of pest sightings in operational areas of the vessel. Ensure the IPM plan includes provisions for training of crew members in charge of log completion. Record the time of the active monitoring inspections in the log.
