On 14 July 2023 (Voyage #138) the standardized 24-hour AGE report did not display the correct number of AGE cases for crew. The AGE log reflected that there was one crew member with AGE symptoms; however, this case was not properly recorded on the standardized 24-hour AGE report that was submitted.

Recommendation:
Ensure the AGE report contains the total numbers of reportable cases of AGE among crew members, including those who have disembarked because of illness?even if the number is 0 (zero reporting).

Violation:
The August 2023 Microbiologic Sampling Report was written incorrectly for the collection time and E. coli results column. The E. coli column was missing whether the results were 'A' Absent or 'P' Present. Additionally, there were no collection times recorded. Staff corrected the log to reflect collection times and the presence of E. coli.

|
Recommendation:
Collect and analyze a minimum of four potable water samples per month for the presence of E. coli. Collect samples from the forward, aft, upper, and lower decks of the vessel. Change the sample sites each month so that all of the potable water distribution system is effectively monitored. Conduct follow-up sampling for each positive test result. Maintain sample results onboard the vessel for at least 12 months and ensure they are available for review during inspections.

Recommendation:
Stripe or paint potable water lines either in accordance with ISO 14726 (blue/green/blue) or blue only. Stripe or paint distillate and permeate lines directed to the potable water system in accordance with ISO 14726 (blue/gray/blue). Ensure no other lines have the above color designations. Stripe or paint these lines at 5 meters (15 feet) intervals and on each side of partitions, decks, and bulkheads, except where decor would be marred by such markings, including potable water supply lines in technical lockers. Do not stripe or paint potable water lines after reduced pressure assemblies as potable water.

The crew member then stopped plating the meat, so he could assist in the serving line. The crew member then went back to placing the meat in the same serving dish without taking any temperatures.

Recommendation:
Cook raw animal foods such as eggs, fish, meat, and poultry?and foods containing these raw animal foods?to heat all parts of the food to a temperature and for a time that complies with one of the following methods based on the food being cooked: (1) 63C (145F) or above for 15 seconds for fish, meat, and pork, including game animals commercially raised for food and game animals under a voluntary inspection program.

When the Integrated Pest Management (IPM) sighting logs were reviewed, there were no entries for any pest sightings at Wheezy's Freezies Bar in 2023. Staff stated they would investigate where the disconnect occurred.

Ensure the supervisor or person in charge of IPM operations on the vessel demonstrates to the VSP?during inspections and on request?knowledge of IPM operations, and is able to demonstrate this knowledge by compliance with Section 8 of the VSP 2018 Operations Manual or by responding correctly to the inspector?s questions as they relate to the specific operation, and properly trains employees to comply with Section 8 of the VSP 2018 Operations Manual as it relates to their assigned duties.
