View/Print Summary Report  |  View/Print Corrective Action Statement
Item No.: 01
Site: Medical-
Violation: The 24-hour report submitted did not include the reportable acute gastroenteritis (AGE) crew member who had been released from medical isolation during voyage segment AMR-060F. The report had 680 total passengers, 408 total crew members, and zero reportable cases for both passenger and crew.
Recommendation: Ensure the master, medical staff, or other designated staff of a vessel destined for a U.S. port from a foreign port submits at least one standardized AGE report based on the number of reportable cases in the AGE log to VSP no less than 24 hours-but not more than 36 hours-before the vessel's expected arrival at the U.S. port.
Item No.: 10
Site: Recreational Water Facilities-Swimming Pool
Violation: There was no hair and lint strainer for the facility, however records indicated this strainer was cleaned and disinfected at least weekly. The only filtration was from the two cartridge filters located after the pumps.
Recommendation: Clean, rinse, and disinfect the hair and lint strainer and hair and lint strainer housing on all RWFs weekly. Ensure disinfection is accomplished with an appropriate halogen-based disinfectant. At a minimum, use a 50-ppm solution for 1 minute, or equivalent CT value. Maintain records on all inspection and cleaning procedures. Ensure that if there is not a hair and lint strainer but there is a filter before the pump, this filter must be cleaned, rinsed, and disinfected weekly. Add a hair and lint strainer to the swimming pool's recirculation system.
Item No.: 10
Site: Recreational Water Facilities-Swimming Pool
Violation: The turnover rate was 2.5 hours which is more than required time for an interactive recreational water facility of its depth. Re-design of this facility is a project for the ship's September/October 2023 dry dock.
Recommendation: For RWFs with skim gutters, ensure that the fill level of the RWF is to the level of the skim gutters. Design recirculation systems and equipment, including chemical control equipment, UV disinfection systems, filter, and pumps to maintain adequate water chemistry control while operating at the following minimum turnover rates: interactive RWF or activity pool greater than 610 millimeters (24 inches) deep: 2 hours.
Item No.: 10
Site: Recreational Water Facilities-Whirlpool Spas
Violation: The port and starboard whirlpool spas did not have separate monitoring for chlorine and pH. The electronic data logger measurements were combined in one reading. Staff explained the starboard whirlpool spa is to be removed during the ship's September/October 2023 dry dock.
Recommendation: Install chart recorders or electronic data loggers with certified data security features that record pH and halogen measurements for each individual RWF. Ensure the sample line for the analyzer probe (monitoring) is either directly from the RWF or on the return line from each RWF and before the compensation tank. Install appropriate sample taps for analyzer calibration. In the event of equipment failure, measure free residual halogen and pH by a manual test kit at the RWF or return line at least hourly for whirlpool spas, spa pools, children's pools, and wading pools and every 4 hours for all other RWFs. Record manual readings on a chart or log, retain for at least 12 months, and ensure that they are available for review during inspections. Complete repairs on malfunctioning halogen analyzer-chart recorders within 30 days of equipment failure. Provide an audible alarm in a continuously occupied watch station to indicate low and high free halogen and pH readings in each RWF. Ensure that manual samples from the RWF tub are compared to the analyzer samples in the pump room to assess potential water quality differences in the RWF.
Item No.: 20
Site: Galley-Bakery
Violation: The dough sheeter belt was in disrepair.
Recommendation: Design and construct food contact surfaces of equipment and utensils to be durable and to retain their characteristic qualities under normal use conditions. Ensure multiuse food-contact surfaces are smooth.
Item No.: 22
Site: Galley-Potwash
Violation: Some of the potwash machine's upper and lower spray arm nozzles were clogged with food debris. Crew started cleaning the spray arms immediately.
Recommendation: Ensure warewashing machines, drainboards, and the compartments of sinks, basins, or other receptacles used for washing and rinsing equipment, utensils, or raw foods, or laundering wiping cloths are cleaned: (1) before use; (2) throughout the day at a frequency necessary to prevent recontamination and accumulation of excessive debris, and to ensure that the equipment performs its intended function; (3) at least every 24 hours (if used).
Item No.: 33
Site: Galley-Walk-in Refrigerator #16
Violation: Water leaked slowly from the deckhead. No food was impacted.
Recommendation: Ensure decks, bulkheads, and deckheads in food preparation, warewashing, pantries, and storage areas are maintained in good repair.
Item No.: 39
Site: Galley-Hot Galley Braising Pans
Violation: Four ants were on the bulkhead behind the right two braising pans. The area was in operation during the inspection.
Recommendation: Effectively control the presence of insects, rodents, and other pests to minimize their presence in the food storage, preparation, and service areas and warewashing and utensil storage areas aboard a vessel.
Item No.: 44
Site: Recreational Water Facilities-
Violation: The person in charge of recreational water operations was not following applicable VSP 2018 Operations Manual requirements. For example: (1) There was no hair and lint strainer for the swimming pool, however records indicated this strainer was cleaned and disinfected at least weekly; (2) The turnover rate for the swimming pool was 2.5 hours which is more than required time for an interactive recreational water facility of its depth; and (3) The port and starboard whirlpool spas did not have separate monitoring for chlorine and pH. The electronic data logger measurements were combined in one reading.
Recommendation: Ensure the supervisor or person in charge of recreational water operations on the vessel demonstrates to VSP-during inspections and on request - knowledge of recreational water operations, is able to demonstrate this knowledge by compliance with Section 6 of the VSP 2018 Operations Manual or by responding correctly to the inspector's questions as they relate to the specific operation, and properly trains employees to comply with Section 6 of the VSP 2018 Operations Manual as it relates to their assigned duties.
