Medical-Acute Gastroenteritis (AGE) Food and Beverage Questionnaire

Violation:
The AGE log stated that a nonfood employee had AGE symptom onset beginning at 0020 on October 5 2024; however, the food and beverage questionnaire stated this nonfood employee was in the crew mess for breakfast, lunch, and dinner on the same day. It was determined by staff and the inspector that the form was filled out incorrectly and the proper isolation protocols took place.

Recommendation:
Distribute questionnaires detailing activities and meal locations for the 72 hours before illness onset to all passengers and crew members who are AGE CASES. Ensure that the self-administered questionnaires contain all of the data elements that appear in the questionnaire found in Annex 13.2.2. Ensure that completed questionnaires are maintained with the AGE surveillance log.

Potable Water-Distant Point Chart

Violation:
The distant point chart recorder had several fluctuations in halogen residual levels; however, there were no notations on the chart explaining these fluctuations. On 02 May 2024, the chart flat lined with a halogen residual level of 3 ppm for 4 hours but no notations were on the chart. It was determined by staff and the inspector that potable water was being transferred to multiple tanks causing halogen levels to vary due to flow differentials. The actual residual levels were verified with a manual log used consistently as a backup to the chart recorder. Additionally, the chart did not have a supervisor initials for validation.

Recommendation:
Change, initial, and date halogen analyzer-chart recorder charts daily. Ensure the charts contain notations of any unusual events in the potable water system. If electronic data loggers are used in lieu of chart recorders, record notations of any unusual events in the potable water system log.

Potable Water-Pipe Disinfection Log

Violation:
On 14 and 25 February, 25 March, 16, 22, and 28 April of 2024, the pipe disinfection log stated multiple 4 x 4 inch pieces of joint pipe were disinfected for 50 ppm for three hours instead of four hours. The staff member stated he oversaw the disinfection process, which was completed by a new employee, it was a clerical error in the disinfection log, and they stated the proper disinfection concentration and required contact time when asked by the inspector.

Recommendation:
Maintain documentation of all inspections, maintenance, cleaning, and disinfection for 12 months and ensure it is available for review during inspections. Ensure records include a method of disinfection, concentration and contact time of the disinfectant, and a halogen value of less than or equal to 5 ppm before the tank is put back into service.

Violation:
The hotel management, nontestable, cross-connection control log stated the backflow prevention devices were continuous pressure devices; however, many of the devices were noncontinuous pressure devices.

Recommendation:
Ensure the cross-connection control program includes at a minimum: a complete listing of cross-connections and the backflow prevention method or device for each, so there is a match to the plumbing system component and location. Include air gaps on the listing. Ensure a schedule for inspection frequency is set. Maintain a log documenting the inspection and maintenance in written or electronic form, and ensure it is available for review during inspections.

Design and construct nonfood-contact surfaces of equipment and utensils to be durable and to retain their characteristic qualities under normal use conditions. Ensure nonfood-contact surfaces are free of unnecessary ledges, projections, and crevices, and designed and constructed to allow easy cleaning and to facilitate maintenance.
