Item No.: 02
Site: Medical-Acute Gastrointestinal (AGE) Case Identification
Violation: There were no records of public vomiting incidents (PVI) or public diarrhea incidents (PDI) communicated between housekeeping and medical staff for further evaluation to prevent the spread of gastrointestinal disease. Recent housekeeping records in 2024 showed: (1) Seven PVI and PDI on 11 October; (2) One PDI on 5 October; (3) One PDI and one PVI on 3 October; (4) One PDI on 24 September; (5) One PVI and one public body fluid incident on 21 September; and (6) Two PVI on 4 September. Medical staff showed the inspector their incident records, which did not include receiving PVI or PDI information for follow-up when guests were identified. Nor had medical staff received emails concerning these incidents.
Recommendation: Ensure a reportable case of AGE is a case reported to the master of the vessel, the medical staff, or other designated staff by a passenger or a crew member and meets the case definition based on the following symptoms: (1) Diarrhea (three or more episodes of loose stools in a 24 hour period or what is above normal for the individual. See section 4.1.2.1.3, illness symptoms); or (2) vomiting and one additional symptom including one or more episodes of loose stools in a 24-hour period, or abdominal cramps, or headache, or muscle aches, or fever (temperature of greater than or equal to 38C [100.4F]). Ensure medical records document evaluations for public vomit/diarrhea incidents when the individuals involved are identified. Proper evaluation should include determining if the individuals meet the definition of an AGE reportable case and assessing the need for isolation.

Site: Buffet-Deck 11 - Great Outdoor / Hot Entrees
Violation: The passenger service line countertop was cracked and had open seams which created a difficult to clean surface.
Recommendation: Ensure nonfood-contact surfaces of equipment that are exposed to splash, spillage, or other food soiling or that require frequent cleaning are constructed of a corrosion-resistant, nonabsorbent, and smooth material. Design and construct nonfood-contact surfaces of equipment and utensils to be durable and to retain their characteristic qualities under normal use conditions. Ensure nonfood-contact surfaces are free of unnecessary ledges, projections, and crevices, and designed and constructed to allow easy cleaning and to facilitate maintenance.

Site: Buffet-Deck 11 - Garden Cafe (Portside)
Violation: A bottle of hypochlorous acid solution at the aft waiter station measured 10 ppm chlorine using a test strip.
Recommendation: Ensure sanitizing solutions are used with the following concentrations: A chlorine solution with a concentration between 50 mg/L (ppm) and 200 mg/L (ppm).

Site: Pantry-Deck 11 - Topsider
Violation: A waste receptacle was open and had food debris inside. The area was not in operation during the inspection.
Recommendation: Cover receptacles and waste-handling containers when not in continuous use and after they are filled.

Site: Buffet-Deck 11 - Garden Cafe (Portside)
Violation: Multiple passenger buffet service lines had low light intensity where food was out for consumer self-service. The light intensity measured from 121-151 lux at the omelet stations, cold cuts, cereals, pastries, and hot entrees lines.
Recommendation: Ensure the light intensity is at least 220 lux (20 foot candles) on food preparation surfaces, and at a distance of 75 centimeters (30 inches) above the deck in food preparation areas, handwashing facilities, warewashing areas, equipment, and utensil storage, pantries, toilet rooms, and consumer self-service areas.

Violation: Four tiny flies were near the bread and sauce station. The area was in operation. The food items were discarded immediately.
Recommendation: Effectively control the presence of insects, rodents, and other pests to minimize their presence in the food storage, preparation, and service areas and warewashing and utensil storage areas aboard a vessel.

Recommendation: Ensure the supervisor or person in charge of housekeeping operations on the vessel demonstrates to the VSP?during inspections and on request ? knowledge of housekeeping operations, is able to demonstrate this knowledge by compliance with section 9 of the 2018 VSP Operations Manual of these guidelines or by responding correctly to the inspector?s questions as they relate to the specific operation, and properly trains employees to comply with section 9 of the 2018 VSP Operations Manual as it relates to their assigned duties. Communicate all PVI/PDI incidents with medical for appropriate follow-up with the ill passengers.
