Site: Potable Water-Mineralizer #2
Violation: The mineralizer outlet pipe was striped blue/green/blue for potable water before the chlorine injection point.
Recommendation: Stripe or paint potable water lines either in accordance with ISO 14726 (blue/green/blue) or blue only. Stripe or paint distillate and permeate lines directed to the potable water system in accordance with ISO 14726 (blue/gray/blue). Ensure no other lines have the above color designations. Stripe or paint these lines at 5 meters (15 feet) intervals and on each side of partitions, decks, and bulkheads, except where decor would be marred by such markings, including potable water supply lines in technical lockers.

Site: Medical-Records: Food Employee
Violation: During voyage 26839, from 25 October to 06 November 2022, a food handler had an onset of acute gastroenteritis symptoms on 25 October at 0730, but did not report to the medical facility until 1215. The crew member worked during this time; their timecard indicated they started work at 0800. A food assessment was completed and the employee received disciplinary action.
Recommendation: When food employees meet the case definition for AGE, ensure the following actions are taken: (1) isolate in cabin or designated restricted area until symptom-free for a minimum of 48 hours; (2) follow-up with and receive approval by designated medical personnel before returning crew to work; (3) document date and time of last symptom and clearance to return to work. Ensure the food employee's supervisor or person in charge conducts an assessment of food prepared or served by the food employee while symptomatic and take appropriate corrective actions. Document corrective actions taken as a result of the assessment.

Violation: The consumer advisory above the salmon did not include the specific disclosure statement that salmon is served raw or undercooked.
Recommendation: Ensure the consumer advisory disclosure is on a sign describing the animal-derived foods (e.g., ?oysters on the half-shell,? ?hamburgers,? ?steaks,? or ?eggs?); AND that they can be cooked to order or may be served raw or undercooked; AND a statement indicating that consuming raw or undercooked meats, seafood, shellfish, eggs, milk, poultry may increase your risk for foodborne illness, especially if you have certain medical conditions.

Violation: The left interior edge of two flat top grills were in disrepair and did not have a smooth, easily cleanable surface.
Recommendation: Ensure multiuse food-contact surfaces are: (1) Smooth; (2) Free of breaks, open seams greater than 0.8 mm (1/32 inch), cracks, chips, inclusions, pits, and similar imperfections; (3) Free of sharp internal angles, corners, and crevices; (4) Finished to have smooth welds and joints; and (5) Accessible for cleaning and inspection by one of the following methods without being disassembled, by disassembling without the use of tools, or by easy disassembling with the use of handheld tools commonly available to maintenance and cleaning personnel.

Violation: The bulk milk container dispensing tube was not cut on the diagonal.
Recommendation: Cut bulk milk dispensing tubes 45 degrees on the diagonal, leaving no more than 25 mm (1 inch) protruding from the chilled dispensing head.

Violation: The hood-type potwash machine leaked excessive water onto the deck when opened after a wash cycle. The water pooled on the deck between the potwash machine and the scupper drain.
Recommendation: Maintain warewashing equipment in good repair and proper adjustment, including: (1) warewashing equipment is maintained in a state of repair and condition that meets the standards of the materials, design, and construction of these guidelines; (2) water pressure and water temperature-measuring devices are maintained in good repair and accurate within the intended range of use.
