Violation: THE QUESTIONNAIRE USED FOR CREW AND PASSENGERS WITH GASTROINTESTINAL ILLNESS DID NOT COLLECT INFORMATION ON PRE-CRUISE ACTIVITIES, SHORE EXCURSIONS, AND EATING/DRINKING ASHORE.
Recommendation: Ensure that questionnaires containing all of the data elements that appear in the questionnaire found in Annex 13.2 and detailing activities and meal locations for the 72 hours before illness onset are distributed to all passengers and crew members who meet the case definition for gastrointestinal illness.

Violation: LINES THAT WERE NOT POTABLE WATER WERE PAINTED BLUE IN THE CREW POOL PUMP ROOM.
Recommendation: Ensure that potable water piping and fittings are painted or stripped auxiliary blue, or in accordance with ISO 14726, at 5 m (15 feet) intervals on each side of partitions, decks, and bulkheads in all spaces, except where the decor would be marred by such markings.

Violation: THE COMPREHENSIVE CROSS-CONNECTION CONTROL PROGRAM WAS NOT A COMPLETE LISTING. HOSE CONNECTIONS IN THE POOL/WHIRLPOOL PUMP ROOMS, AND CONNECTIONS SUPPLYING THE CREW AND PASSENGER LAUNDERETTES WERE NOT LISTED.
Recommendation: Ensure that the cross-connection control program includes at minimum: a complete listing of cross-connections and the backflow prevention method/device for each, so that there is a match to the plumbing system component and location, and an inspection frequency. Ensure that air-gaps are include din the listing.

Violation: A BACKFLOW PROTECTION DEVICE WAS NOT PROVIDED ON THE POTABLE WATER LINE SUPPLYING THE HOSE CONNECTION IN THE PUMP ROOM FOR MICKEY'S POOL.
Recommendation: Ensure that an appropriate backflow protection device is provided for the potable water line supplying this connection.

Violation: THE DEPTH MARKERS COULD NOT BE SEEN FROM INSIDE THE POOL. A SAFETY SIGN WAS NOT AVAILABLE.
Recommendation: Ensure that the depth of each pool is prominently displayed so that it can be seen from the deck and in the pool. Ensure that depth markers are labeled in feet or meters, or both,. Additionally, ensure that depth markers are installed for every 1 m (3 feet) in change of depth. Ensure that safety signs are provided for public swimming pools and whirlpool spas.

Violation: THERE WAS SEAWEED STORED BELOW RAW FISH IN THE WALK-IN REFRIGERATOR. THE SEAWEED IS USED FOR GARNISH. THIS WAS CORRECTED DURING THE INSPECTION.
Recommendation: Ensure that ready-to-eat food is stored above raw animal products. Ensure that food is protected from cross-contamination by: (1) Physically separating raw animal foods during storage, preparation, holding, and display from raw ready-to-eat food including other raw animal food such as fish for sushi or molluscan shellfish, or other raw ready-to-eat food such as vegetables, and cooked ready-to-eat food; so products do not physically touch, and so as to prevent dripping of one product into another.

Recommendation: Ensure that materials used in the construction of multiuse utensils and food-contact surfaces of equipment are: (1) Durable, corrosion-resistant, and nonabsorbent; (2) Sufficient in weight and thickness to withstand repeated warewashing; (3) Finished to have a smooth, easily cleanable surface; and (4) Resistant to pitting, chipping, crazing, scratching, scoring, distortion, and decomposition.

WATER FLOWS OVER THE SURFACE OF THE FASTENERS AND THESE SLOTTED FASTENERS WERE DIFFICULT TO CLEAN. THE FASTENERS IN THIS MACHINE WERE REPLACED DURING THE INSPECTION. THESE ICE MACHINES WERE USED THROUGHOUT FOOD SERVICE AND HOUSEKEEPING AREAS ON THE VESSEL.
Recommendation: Replace the slotted fasteners with smooth, low-profile, non-corroding fasteners.

Recommendation: Ensure that nonfood-contact surfaces of equipment that are exposed to splash, spillage, or other food soiling or that require frequent cleaning are constructed of a corrosion-resistant, nonabsorbent, and smooth material. Ensure that nonfood-contact surfaces are free of unnecessary ledges, projections, and crevices, and designed and constructed to allow easy cleaning and to facilitate maintenance.

Recommendation: Ensure that the sanitary wipes provided in the toilet room are moist. Ensure that a complete and detailed policy is provided in writing at the center.
