Violation:
There was a non-continuous pressure backflow preventer installed on the hot and cold potable water supply pipes to the sink opposite the dishwash machine in the Flounders section of the nursery. The sink tap had a spray nozzle and hose after the device. These devices were not on the cross-connection control program list.

Recommendation:
Ensure that a continuous pressure-type backflow preventer is installed when a valve is located downstream from the backflow preventer. Ensure that the backflow preventers on the sink are included in the cross connection control program.

Violation:
A major modification made to the equipment and system of halogenation and filtration in all the whirlpool pump rooms included chlorine dosing of returned water from the whirlpool spa overflow in the compensation tank directly with no filtration until after passing thru the compensation tank and being pumped to a filter. Following filtration, there were ultraviolet light disinfection units installed, then heaters, then a return of this water to the whirlpool spa tubs. A potable water pipe meant for topping off the compensation tank had a direct connection to the pipe returning the filtered water to the whirlpool tub. The pipe leading to the compensation tank had the valve in the closed position. Lastly, the chlorine sample point was located after the compensation tank and before the filtration, UV treatment, and heater components.

Recommendation:
Ensure that the halogenation of the whirlpool water occurs after filtration, ultraviolet disinfection and heating of the water system. Organic material, ultraviolet light and heating will all reduce the amount of halogen in the water.
Ensure that the chlorine sample point is located prior to halogenation of the return water so that the sample will represent the water in the whirlpool. Ensure that when the whirlpool is in operation, the valves on the potable water filling line are positioned so that potable water will enter the compensation tank and be halogenated prior to entering the whirpool.

Violation:
Although there is documentation when there is or is not a cabin mate when crew gastrointestinal illness (GI) cases are reported, there is only documentation if there is an immediate contact for crew GI cases. There is no record made to indicate there are no immediate contacts.

Recommendation:
Ensure that the documentation includes whether or not the ill crew member has close contacts to be tracked for symptoms.

The record of interviews for asymptomatic cabin mates and immediate contacts for the reported crew gastrointestinal illness (GI) cases was consistently done within 24 hours after onset of the ill crew members symptoms, but was not in the 48 hour after onset period. Additionally, when completing the form on isolation and release of GI cases and roommates/immediate contacts there were dates given for the asymptomatic cabin mates and immediate contacts under the title last GI symptom and release from isolation. These fields were completed on the form even though the roommate or immediate contact had no symptoms.

Recommendation:
Ensure that medical staff conduct daily verbal interviews with asymptomatic crew until 48 hours after onset of the ill crew members symptoms. Ensure that the forms are clear as to the symptom status of the asymptomatic contacts of crew members with gastrointestinal illness.

Ensure that decks, bulkheads, and deckheads in food preparation, warewashing, pantries, and storage areas are constructed and maintained for easy cleaning. Ensure that decks, bulkheads, deckheads, and attached equipment in food preparation, warewashing, pantries, and storage areas, are cleaned as often as necessary.
