Site: Potable Water-Crew Whirlpool Fill Line
Violation: The distance between the reduced pressure backflow prevention assembly and the drain funnel was less than two times the diameter of the vent.
Recommendation: Ensure that air gaps are at least twice the diameter of the delivery fixture opening and a minimum of 3 cm (1 inch).

Site: Potable Water-
Violation: The hose connections by the handwash stations in the garbage room and engine change room were not listed in the cross-connection control program. These connections were adequately protected.
Recommendation: Ensure that the cross-connection control program includes a complete listing of cross-connections and the backflow prevention method/device for each, so that there is a match to the plumbing system component and location, and an inspection frequency.

Site: DO NOT USE (whirlpool/spa)-
Violation: The free bromine residual was less than 4.0 ppm for more than two hours at the mid-port whirlpool on December 29, 2008 and January 2 and 4, 2009. The free bromine residual was less than 4.0 ppm for more than two hours at the mid-starboard whirlpool on December 30, 2008 and at the aft-port whirlpool on January 9, 2009. There was no explanation on the charts.
Recommendation: Ensure that whirlpool spas are maintained at a free residual chlorine of >3.0 mg/L (ppm) and <10 mg/L (ppm), or a free residual bromine of >4.0 mg/L (ppm) and <10 mg/L (ppm).

Violation: The main drain was damaged. There was no water in the whirlpool.
Recommendation: Repair the main drain prior to opening the whirlpool.

Violation: The pool section of this facility was deeper than one meter. There was no shepherd's hook or flotation device.
Recommendation: Provide a shepherd's hook and flotation device.

Violation: The shelving unit had numerous small openings and other features, making it difficult to clean.
Recommendation: Ensure that nonfood-contact surfaces are free of unnecessary ledges, projections, and crevices, and designed and constructed to allow easy cleaning and to facilitate maintenance.

Violation: There were two folding card tables which were rusting and had damaged edges. Per staff the tables were used in the dinning area to store soiled items.
Recommendation: Ensure that nonfood-contact surfaces of equipment and utensils are designed and constructed to be durable and to retain their characteristic qualities under normal use conditions. Ensure that nonfood-contact surfaces of equipment that are exposed to splash, spillage, or other food soiling or that require frequent cleaning are constructed of a corrosion-resistant, nonabsorbent, and smooth material.

Violation: There was a wiping cloth stored in an ashtray and placed inside the deckhead ventilation diffuser panel.
Recommendation: Ensure that wiping cloths that are in use for wiping food spills are used for no other purpose.

Site: Housekeeping-Lido Aft passenger Ladies Toilet Room
Violation: The tissue dispenser to exit the toilet room was mounted on the inner door. The inner door no self-closing and the dispenser was not visible or accessible. There was no means to exit the outer door without the use of bare hands.
Recommendation: Equip public toilet facilities so that persons exiting the toilet room are not required to handle the door with bare hands. This may be accomplished by methods such as locating paper towel dispensers at sinks and waste containers near the room door, installing mechanically operated doors, door removal, or other effective means.
