Site: Potable Water-Cross-connection Control Program
Violation: The reduced pressure backflow prevention device on the small starboard evaporator had not been tested in the last 12 months. Staff indicated that this evaporator was not in use and could not be tested.
Recommendation: Ensure that backflow prevention devices requiring testing, for example reduced pressure backflow preventer and double check valves with test cocks, are inspected and tested with a test kit at least annually and that the test results showing the pressure differences on both sides of the valves are maintained for each device.

Site: Potable Water-Cross-connection Control Program
Violation: Numerous cross-connections were not included on the cross-connection control program. Examples include the atmospheric vacuum breaker on the garbage room trash can wash, continuous pressure unit on the garbage room chemical feed line, and the continuous pressure unit on the spa steam generator.
Recommendation: Ensure the cross connection control program has a current updated list of the backflow prevention devices.

Site: Potable Water-Cross-connection Control Program
Violation: Some of the cross connections on the list gave a serial or unit number, but not the method/dievice.
Recommendation: Ensure that the cross-connection control program includes at minimum: a complete listing of cross-connections and the backflow prevention method/device for each, so that there is a match to the plumbing system component and location, and an inspection frequency. Ensure that air-gaps are included in the listing.

Site: Recreational Water Facilities-Oasis Swimming Pool
Violation: There was no indication that the drain cover was anti-entrapment. A purchase order (511-1-03714) for anti-entrapment drain covers was in place.
Recommendation: Ensure that anti-entrapment drain covers or other drains that prevent entrapment hazards as specified in U.S. Consumer Product Safety Publication 363-009801 (dual drains/channel drains) are provided on swimming pools and whirlpool spas. Ensure that drain covers have either the plumbing/engineering approving organization stamp and flow rate affixed to the cover, or that a letter is maintained aboard the vessel certifying that the cover meets the safety requirements outlined in AMSE/ANSI A112.19.8M or an equivalent standard.

Violation: The dispensing tube for the bulk milk unit was not cut an angle.
Recommendation: Ensure that the bulk milk container dispensing tube is cut on the diagonal leaving no more than 25mm (1 inch) protruding from the chilled dispensing head.

Violation: Slotted fasteners were noted in the ice/water fall area of the Cornelius 600 ice machine.
Recommendation: Remove these slotted fasteners and replace with a smooth non-slotted non-corroding low profile fasteners.

Violation: The IPM plan was last revised in September 2004. There was no indication that the plan has been evaluated for effectiveness in the last two years.
Recommendation: Ensure that the vessel's Integrated Pest Management Plan is evaluated for effectiveness every 2 years or whenever there is a significant change in the vessel's structure such as a renovation or operation.
