Item No.: 01
Site: Medical-Medical Reporting - 4-hour Update Report
Violation: For the voyage ending 09/21/2007, one passenger GI case reported to the medical infirmary after the submission of the 24-hour report, but greater than 4-hours before the ship's scheduled arrival at Honolulu. A 4-hour update report was not submitted.
Recommendation: If the number of cases changes after submission of the initial report, ensure that an updated report is submitted no less than 4 hours before the vessel's arrival at the U.S. port.

Site: Potable Water-Bunkering Charts
Violation: Charts dated from 29 August 2007 to present indicated halogen levels to be below the required 2.0 ppm minimum during bunkering. Manual halogen testing logs indicated that the levels were between 2.1 ppm and 2.3 ppm, however, the manual tests were not performed hourly. Charts dated prior to 29 August 2007 also showed halogen levels below the required minimum, however, these charts had hourly manual tests recorded directly on the charts confirming that the halogen levels were between 2.1 ppm and 2.5 ppm. The chart recorder pin was out of alignment.
Recommendation: Ensure that the halogen analyzer-chart recorder is properly maintained, operated, and calibrated daily in accordance with the manufacturer's instructions. Ensure that a manual comparison test is conducted daily to verify calibration and that calibration is made whenever the manual test value is more than 0.2 ppm higher or lower than the analyzer reading.

Site: Potable Water-Far Point Chart Recorder
Violation: At the time of the inspection, the far point analyzer reading was 0.40 ppm free chlorine residual and the manual test result was 0.25 ppm. The chart was logging 0.70 ppm. The chart recorder pin was out of alignment.
Recommendation: Ensure that the halogen analyzer-chart recorder is properly maintained, operated, and calibrated daily in accordance with the manufacturer's instructions. Ensure that a manual comparison test is conducted daily to verify calibration and that calibration is made whenever the manual test value is more than 0.2 ppm higher or lower than the analyzer reading.

Site: Potable Water-Cross-connection Control Program
Violation: The cross-connection control program's backflow preventer listing was not complete and/or accurate. The two evaporator high saline discharge line reduced pressure principle backflow preventers and the preventers installed on the spa hair sinks were not listed. The backflow prevention units listed for the whirlpool technical room, Sabatini's pot wash scraping hoses, and garbage room were not the types recorded as being installed in these locations.
Recommendation: Ensure that the cross-connection control program includes at minimum: a complete listing of cross-connections and the backflow prevention method/device for each, so that there is a match to the plumbing system component and location, and an inspection frequency. Ensure that air-gaps are included in the listing.

Site: Potable Water-Tank Maintenance
Violation: The tank maintenance records did not indicate that the halogen level was verified to be below 5.0 ppm before tanks were placed back on line.
Recommendation: Ensure that the disinfected parts of the system are flushed with potable water until the free residual halogen is at or below 5.00 mg/L (ppm).

Site: Potable Water-Spa Salon
Violation: Only one half of the Watts N9 series backflow preventer was installed on the men's steam room unit.
Recommendation: Replace the backflow preventer.

Site: Galley-Main Galley - Potwash Area
Violation: During active operation of the potwash, a significant amount of water was exiting the atmospheric vent of the atmospheric vacuum breaker.
Recommendation: Repair or replace the backflow prevention device.

Site: DO NOT USE (whirlpool/spa)-Swimming Pool/Whirlpool Spa Charts
Violation: The charts were scaled 0-5 ppm, however the analyzer units were calibrated for 0-10 ppm. Daily manual shock halogenation tests (recorded on the charts) indicated 10-13 ppm, while the chart readings never exceeded 5.0 ppm. This was noted in the last inspection.
Recommendation: Provide charts that record the halogen levels within the range of use.

Violation: The mounted temperature gauges on two of the bain maries were missing protective covers, making cleaning difficult.
Recommendation: Replace missing protective covers to mounted temperature gauges.

Violation: The rear panel of one of the flat grills was misaligned, leaving a gap, making cleaning difficult.

Violation: The water temperature at the handwash station was measured in excess of 128F. The water was too hot for proper handwashing.
Recommendation: Ensure that handwashing sinks are equipped to provide water at a temperature of at least 43C (110F) through a mixing valve or combination faucet. For handwash sinks with electronic sensors, where the user cannot make temperature adjustments, ensure that the temperature provided to the user does not exceed 52C (125F).

Violation: The technical compartment had a large opening where the utility lines passed through the bulkhead. This feature makes cleaning difficult.
Recommendation: Seal the hole in the rear panel of the technical compartment.

Violation: The flexible tubing waste drain line from the bain maries was soiled with greater than one day's dirt and debris.
Recommendation: Ensure that decks, bulkheads, deckheads, and attached equipment in food preparation, warewashing, pantries, and storage areas, are cleaned as often as necessary.

Violation: Some sections of the coving at the deck-bulkhead juncture were missing, leaving areas where food and debris could collect, making cleaning difficult.
Recommendation: Ensure that bulkhead and deck junctures are coved (including galleys, pantries, deck/counter junctures at buffets, bars, waiter stations, and dining room work counters).

Violation: The condensate collection pan was not draining and the pan was completely full of condensate. This was corrected at the time of the inspection..

Violation: The frame for the thermo wheel was severely rusted. Staff stated that this is an item to be corrected during the next dry dock.
