Violation: THE FECAL ACCIDENT LOG DOES NOT LIST THE LOCATION OF THE INCIDENTS.
Recommendation: Ensure the location of any fecal accidents are documented.

Violation: THE HALOGENATION CONTACT TIME WAS NOT RECORDED ON THE TANK CLEANING LOG.
Recommendation: Ensure the halogenation contact time is recorded on the tank cleaning log.

Violation: THE BACKFLOW PREVENTION DEVICE WAS MISSING FROM THE POTABLE WATER OUTLET LINE USED TO FILL THE CHEMICAL TANKS IN THE ENGINE ROOM AREA TT 5. THE DEVICE WAS REPLACED DURING THE INVESTIGATION.
Recommendation: Ensure backflow preventers are provided for all potable water outlet lines.

Violation: THE HYDROTHERAPY TUB IS CLEANED WITH MIKRO-BAC II BETWEEN CUSTOMERS. STAFF STATED THAT THE CONTACT TIME FOR THE PRODUCT WAS 2 MINUTES. THE LABEL LISTS A 10 MINUTE CONTACT TIME.
Recommendation: Ensure the recommended contact time for the micro-bac II is followed as directed on the manufacture's label.

Violation: THE DISHWASH ATTENDANT WAS OBSERVED WORKING ON THE CLEAN END OF THE WAREWASHER PLACING A SOILED DISH BACK ON THE DIRTY END FOR REWASHING AND HANDLING THE DISH SPRAYER AT THE DIRTY END OF THE WASHER. THE ATTENDANT THEN WENT TO THE HANDWASH SINK AND TOOK OFF THE RUBBER GLOVES THEN PROCEEDED TO WASH HIS HANDS AND PLACED THE SOILED GLOVES BACK ON AND PROCEEDED TO RACK DISHES ON THE CLEAN END OF THE WASHER AGAIN .
Recommendation: 7.2.3.1.3 Food employees shall clean their hands and exposed portions of their arms immediately before engaging in food preparation including working with exposed food, clean equipment and utensils, and unwrapped single-service and single-use articles and: (1) After touching bare human body parts other than clean hands and clean, exposed portions of arms; (2) After using the toilet room; (3) After coughing, sneezing, using a handkerchief or disposable tissue, using tobacco, eating, or drinking; (4) After handling soiled equipment or utensils; (5) During food preparation, as often as necessary to remove soil and contamination and to prevent cross- contamination when changing tasks; (6) When switching between working with raw food and working with ready-to-eat food; and (7) After engaging in other activities that contaminate the hands.

Violation: SEVERAL ITEMS WERE MARKED OUT OF ORDER WHICH INCLUDED THE AUTO-SHAM, COMBI-OVEN AND BLAST CHILLER. THE REPAIR PARTS HAVE BEEN ORDERED.
Recommendation: 7.4.5.1.1.1 Food-contact equipment shall be maintained in good repair and proper adjustment including: (1) Equipment shall be maintained in a state of repair and condition that meets the materials, design, construction, and operation specifications of these guidelines.

Violation: THE WASH TEMPERATURE GAUGE ON THE GLASSWASHER WAS NOT READABLE DUE TO HEAVY CONDENSATE BUILD UP.
Recommendation: 7.5.2.2.1 Water temperature measuring devices shall be designed to be easily readable.

Violation: THE DISHWASH MACHINE FINAL RINSE ARM WAS NOT ALIGNED SO THAT THE FINAL RINSE COVERED THE WHOLE TRAY OF DISHWARE.
Recommendation: 7.5.4.1.1 Warewashing equipment shall be maintained in good repair and proper adjustment including: (1) Warewashing equipment shall be maintained in a state of repair and condition that meets the standards of the materials, design, and construction of these guidelines. (2) Water pressure, and water temperature measuring devices shall be maintained in good repair and be accurate within the intended range of use.

Violation: THE VENTILATION HOODS WERE SLIGHTLY SOILED WITH GREASE.
Recommendation: 7.5.5.1.3 Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.

Violation: THE HANDWASH SINK AT THE ENTRANCE TO THE CREW MESS WAS BLOCKED.

Violation: THE GROUTING WAS RECESSED ON THE DECKING IN FRONT OF THE SOUP KETTLE STATION MAKING THE DECK DIFFICULT TO CLEAN.
Recommendation: 7.7.4.1.1 Decks, bulkheads, and deckheads in food preparation, warewashing, pantries, and storage areas shall be constructed and maintained for easy cleaning.

Violation: THE PADDED PLASTIC COVERED SURFACES OF THE CHILD ACTIVITY CENTER PLAY AREAS WERE CLEANED WITH MIKRO-BAC II. THE MIKRO-BAC II LABEL STATES THAT THIS PRODUCT SHOULD BE USED ON HARD NON-POROUS SURFACES. THE LABEL LISTS "FOR USE IN HOSPITALS, NURSING HOMES, SCHOOLS, COLLEGES, MEDICAL AND DENTAL OFFICES, AND VETERINARY CLINICS". THE LABEL DOES NOT LIST THIS PRODUCT FOR USE IN CHILD CARE SETTINGS.
Recommendation: Ensure chemicals used for cleaning of plastic surfaces are from an approved source for use in a child care setting.
