Violation: IN DEVELOPING THE CORRECTIVE ACTION STATEMENT FOR THIS INSPECTION, CRITICAL-ITEM DEFICIENCIES (DESIGNATED WITH YES IN CRITICAL COLUMN (WORTH 3 - 5 POINTS), WHETHER DEBITED OR NOT, SHOULD INCLUDE STANDARD OPERATING PROCEDURES AND MONITORING PROCEDURES IMPLEMENTED TO PREVENT THE RECURRENCE OF THE CRITICAL DEFICIENCY. Site: Other-MEDICAL
Violation: THERE WAS NO DOCUMENTATION THAT THE GI ILLNESS REPORT FOR THE CRUISE 1/18/2004 TO 1/25/2004 WAS RECEIVED BY VSP. THE VESSEL'S COMPUTER SYSTEM INDICATED THAT THE REPORT HAD BEEN TRANSMITTED. THE VESSEL DID NOT FOLLOW-UP TO ENSURE THAT THE REPORT HAD SUCCESSFULLY BEEN RECEIVED BY VSP. THERE WAS DOCUMENTATION FROM THE VSP SYSTEM THAT THE PAST REPORTS WERE SENT AND RECEIVED.
Recommendation: Ensure that follow-up is done to ensure that GI Illness reports are successfully received at VSP.

Site: Other-STAFF MESS
Violation: A NON-CONTINUOUS PRESSURE TYPE BACKFLOW PREVENTER WAS INSTALLED ON THE POTABLE WATER SUPPLY LINE TO THE VITALITY JUICE MACHINE.
Recommendation: Install a continuous pressure type backflow preventer on the potable water supply line to the juice machine.

Site: Other-FOOD SERVICE - GENERAL
Violation: THE DISPENSING TRIGGERS ON THE VITALITY JUICE MACHINE WERE NOT DESIGNED FOR SANITARY DISPENSING OF BEVERAGES. THE DISPENSERS IN THE MAIN GALLEY BEVERAGE STATIONS WERE RETROFITTED WITH SANITARY TRIGGERS.
Recommendation: Install sanitary triggers on all juice dispensers. Highest priority should be given to those that are installed in self-service areas such as the crew mess rooms, and passenger self-service areas.

Violation: THE DRINK DISPENSER HAD HEAVY SEALANT APPLIED AT THE BASE OF THE MACHINE WHICH WAS NOT APPLIED EVENLY MAKING THE SURFACE NOT EASILY CLEANABLE.
Recommendation: 7.4.1.1.9 Nonfood-contact surfaces of equipment that are exposed to splash, spillage, or other food soiling or that require frequent cleaning shall be constructed of a corrosion-resistant, nonabsorbent, and smooth material.

Site: Other-MAIN GALLEY
Violation: THE MILK DISPENSING TUBE AND THE ATTACHMENTS FOR ONE OF THE WMF COFFEE MACHINES WERE FOUND SOAKING IN A SMALL BAINS MARIE PAN NEXT TO THE UNIT. IT WAS STATED THAT AFTER THE SOAKING THESE ITEMS WERE SENT TO THE DISHWASH AREA TO BE WASH, RINSED, AND SANITIZED. IT WAS NOT CLEAR WHAT THE CONCENTRATION OF CLEANING LIQUID WAS ADDED TO THE WATER IN THE CONTAINER.
Recommendation: Recommend that the cleaning of these pieces of equipment be done at a designated area designed for cleaning of equipment not next on the beverage line.

Violation: THE TILE AT THE BOTTOM OF SOME BULKHEAD AND DECK JUNCTURES HAD OPEN SEAMS MAKING CLEANING DIFFICULT.
Recommendation: 7.7.4.1.1 Decks, bulkheads, and deckheads in food preparation, warewashing, pantries, and storage areas shall be constructed and maintained for easy cleaning.
