Violation: SOME OF THE PORT MICROBIOLOGIC TEST RESULTS WERE MORE THAN A MONTH OLD AT THE TIME OF BUNKERING.
Recommendation: Ensure that the microbiologic test results of the port water are not more than 1 month old.

Violation: THE MAINTENANCE, CLEANING, AND DISINFECTION RECORDS FOR POTABLE WATER TANKS 1 AND 3 WERE MORE THAN 2 YEARS OLD. POTABLE WATER TANK 6 WAS CLEANED 09/29/2002 BUT THERE WAS NO RECORD OF DISINFECTION. POTABLE WATER TANK 7 PORT WAS WORKED ON 09/29/2002 BUT THERE WAS NO RECORD OF CLEANING OR DISINFECTION.
Recommendation: 5.3.4.1.2 Potable water tanks shall be inspected, cleaned, and disinfected during dry docks and wet docks, or every 2 years, whichever is less. 5.3.4.1.3 Documentation of the cleaning shall be maintained for 12 months and shall be available to the VSP for review during inspections.

Violation: THE CROSS-CONNECTION CONTROL PROGRAM DID NOT LIST CONNECTIONS TO THE POTABLE WATER SYSTEM AND THE FORM OF PROTECTION USED.
Recommendation: 5.7.1.1.3 The vessel shall provide a comprehensive cross- connection control program that provides safe connections to the potable water system through air gaps or appropriate backflow devices at the following locations, if present: (1) Potable water supply lines to swimming pools, whirlpool spas, hot tubs, bathtubs, showers, and similar facilities; (2) Photographic laboratory developing machines and utility sinks; (3) Beauty and barber shop spray-rinse hoses; (4) Potable water faucets where hoses are connected or can be connected by threaded or quick-connect outlets such as those serving tanks containing chlorine and other chemicals, and deck taps; (5) Garbage grinders and pulpers; (6) Mechanical warewashing machines; (7) Hospital and laundry equipment; (8) Air conditioning expansion tanks; (9) Boiler feed water tanks; (10) Fire systems; (11) Toilets; (12) Potable water, bilge, and sanitary pumps that require priming; (13) Freshwater or saltwater ballast systems; (14) Bilge or other waste water locations; (15) International shore connection; and (16) Any other connection between potable and non-potable water systems.

Violation: THE REDUCED PRESSURE BACKFLOW PROTECTION DEVICE FOR THE SPRINKLER SYSTEM WAS ON A WHEELED CART. THIS ONE DEVICE WAS USED FOR 4 SEPARATE CONNECTIONS ON THE VESSEL. AS PER THE STAFF THESE CONNECTIONS WERE ONLY USED DURING DRY DOCKS.
Recommendation: Install a reduced pressure backflow prevention device at each international shore connection for the fire suppression systems including those supplying the four sprinkler connections.

Violation: A CHECK VALVE WAS NOTED ON THE WATER LINE TO ICE MACHINE # 4 INSTEAD OF A VENTED BACKFLOW PREVENTION DEVICE.

Violation: A BACKFLOW PROTECTION DEVICE WAS NOT PROVIDED ON THE POTABLE WATER LINE SUPPLYING THE COFFEE MACHINE AT STATION 6. BACKFLOW PROTECTION DEVICES WERE NOT INSTALLED ON THE POTABLE WATER LINES SUPPLYING THE ICE MACHINE AND COFFEE MACHINE AT STATION 5.
Recommendation: Install the appropriate backflow protection devices on the potable water lines supplying these units.

Site: Other-NEPTUNE PORT SPA
Violation: THE BROMINE RESIDUAL WAS TESTED AT 2.0 PPM. THE WHIRLPOOL WAS OPEN AND TWO INDIVIDUALS USED THIS WHIRLPOOL APPROXIMATELY 10 MINUTES PRIOR TO THE TEST. THE WHIRLPOOL WAS IMMEDIATELY CLOSED AND NETTED.
Recommendation: 6.3.2.1.1 Whirlpool spas shall maintain a free residual chlorine of >3.0 mg/L (ppm) and <10 mg/L (ppm), or a free residual bromine of >4.0 mg/L (ppm) and <10 mg/L (ppm).

Site: Other-WHIRLPOOL SPAS
Violation: A REVIEW OF THE RECORDS NOTED NUMEROUS READINGS OF LESS THAN 4.0 PPM BROMINE WITH NO ACTION TAKEN AND NO INDICATION THAT THE WHIRLPOOLS WERE CLOSED. IN ADDITION, ON NUMEROUS DAYS THE RECORDS SHOWED CLOSING OF WHIRLPOOLS WITH A YELLOW HIGHLIGHT AT A SPECIFIC TIME DURING THE DAY DUE TO LOW RESIDUALS. THE HIGHLIGHT DID NOT CARRY THROUGH TO SUBSEQUENT READINGS THAT WERE BELOW 4.0 PPM. IT WAS NOT CLEAR FROM THE LOGS THAT THE WHIRLPOOLS WERE KEPT CLOSED UNTIL THE LEVEL OF BROMINE WAS ACCEPTABLE.
Recommendation: 6.3.2.1.1 Whirlpool spas shall maintain a free residual chlorine of >3.0 mg/L (ppm) and <10 mg/L (ppm), or a free residual bromine of >4.0 mg/L (ppm) and <10 mg/L (ppm). Provide a clearer description in the logs of the action taken to correct halogen residuals outside of the acceptable range. In addition, have the logs clearly indicate when the whirlpools are closed and when they are open for use.

Violation: THE PROCEDURE FOR BACKWASHING WAS AUTOMATED. THERE WAS NO INDICATION THAT THE BACKWASHING CONTINUED UNTIL THE WATER WAS CLEAR.
Recommendation: 6.3.1.1.4 Granular filters shall be backwashed at least daily until the sight glass indicates a clean flow. The uniformity of the backwash action shall be observed, where possible. Other types of filter media shall be backwashed or cleaned at the frequency specified by the manufacturer.

Recommendation: 7.5.7.3.1 Cleaned equipment and utensils, laundered linens, and single-service and single-use articles shall be stored: (1) In a clean, dry location; (2) Where they are not exposed to splash, dust, or other contamination; and (3) At least 15 centimeters (6 inches) above the deck. Install a splash shield between the handwash sink and the storage shelving.
