Violation: A VACUUM BREAKER UNDER HANDWASHING SINK WAS LEAKING.
Recommendation: Water supply lines and fixtures must be free from leaks and properly installed.

Violation: VACUUM BREAKER AIR RELIEF HOLE ON CARBONATOR WATER SUPPLY LINE WAS PLUGGED MAKING IT INEFFECTIVE.

Violation: MANY OF THE PLASTIC COATED SERVING PANS WERE ABRADED AND PEELING MAKING THEM DIFFICULT TO CLEAN.

Recommendation: Spray-type dishwashing machines shall be maintained in good repair. Machines shall be operated in accordance with manufacturers' instructions, and utensils and equipment placed in the machine shall be exposed to all dishwashing cycles.

Violation: FINAL SANITIZING RINSE ON CONVEYOR DISHWASHER WAS NOT ACTIVATING.
Recommendation: Spray-type dishwashing machines shall be maintained in good repair. Machines shall be operated in accordance with manufacturers' instructions, and utensils and equipment placed in the machine shall be exposed to all dishwashing cycles.

Violation: THE DISHWASHING FINAL RINSE TEMPERATURE MEASURED 228 F WHILE THE ACTUAL TEMPERATURE WAS 192 F.
Recommendation: Machine or water line-mounted, numerically scaled, indicating thermometers, accurate to +3F (+1.5C), shall be provided to indicate the temperature of the water in each tank of the machine and the temperature of the final rinse water as it enters the manifold.

Violation: A FEW PREVIOUSLY CLEANED PLATE COVERS WERE SOILED WITH FOOD RESIDUE.
Recommendation: Food contact surfaces of equipment shall be thoroughly washed, rinsed and sanitized after each use and prior to storage. Any items which are found to be soiled after being washed, must be returned to the warewashing area for immediate re-washing.

Violation: THREE OF THE CUTTING BOARDS WERE SOILED WITH A GREASE FILM.
Recommendation: Food contact surfaces of equipment shall be thoroughly washed, rinsed and sanitized after each use and prior to storage. Any items which are found to be soiled after being washed, must be returned to the warewashing area for immediate re-washing.

Violation: FRYER HEATING ELEMENTS WERE SOILED WITH FOOD AND GREASE RESIDUE.
Recommendation: The food contact surfaces of grills, griddles and similar cooking devices shall be cleaned at least once per day. Examination of the cleaned equipment should be made to determine if any areas require further cleaning and sanitizing.

Violation: THE DISPENSING VALVE ON AN INSULATED BEVERAGE DISPENSER IN CLEAN STORAGE AREA HAD NOT BEEN DISASSEMBLED FOR CLEANING. VALVE PARTS WERE HEAVILY SOILED.
Recommendation: Food contact surfaces of equipment shall be thoroughly washed, rinsed and sanitized after each use and prior to storage. Any items which are found to be soiled after being washed, must be returned to the warewashing area for immediate re-washing. Ensure all food contact surfaces are fully disassembled to facilitate cleaning.

Violation: THE INTERIOR OF THE ELECTRICAL CABINET ADJOINING THE FRYER WAS SOILED WITH GREASE.
Recommendation: Non-food contact surfaces of equipment shall be cleaned as often as is necessary to keep the equipment free of accumulation of dust, dirt, food particles, and other debris.
