Item No.: 3
Site: Other-POTABLE WATER
Violation: THE FAR POINT ANALYZER READ 0.6 PPM WHILE THE MANUAL TEST WAS 1.2 PPM. THE ANALYZER CANNOT BE CALIBRATED WHEN THE pH IS OVER 7.6. THE pH WAS MEASURED AT 7.8 AND COULD NOT BE LOWERED AS THE ACID PUMP COULD NOT SUPPLY ENOUGH VOLUME OF ACID TO LOWER THE pH.
Recommendation: Install a larger capacity pump to control the pH of the water being bunkered or do not bunker in water in ports where the pH is greater than 7.6.

Violation: THERE WERE NO BACKFLOW PREVENTERS INSTALLED TO THE POTABLE WATER CONNECTIONS USED FOR THE DIALYSIS MACHINES.
Recommendation: Install appropriate backflow preventers on these potable water lines. Also when the dental chair is connected to the potable water system ensure that backflow prevention is provided.

Violation: UNDERCOUNTER REFRIGERATOR 4.30A WAS FOUND WITH AN INTERNAL AMBIENT AIR TEMPERATURE OF 60oF. NO POTENTIALLY HAZARDOUS FOOD WAS STORED IN THE REFRIGERATOR.
Recommendation: Refrigerators used to store potentially hazardous foods must be maintained at 40oF (4.5oC) at all times to assure that potentially hazardous foods stored therein stabilize at or below 45oF (7oC).

Violation: UNDERCOUNTER REFRIGERATOR A4 WAS FOUND WITH AN AMBIENT AIR TEMPERATURE OF 70oF. NO POTENTIALLY HAZARDOUS FOOD WAS STORED IN THE REFRIGERATOR.
Recommendation: Refrigerators used to store potentially hazardous foods must be maintained at 40oF (4.5oC) at all times to assure that potentially hazardous foods stored therein stabilize at or below 45oF (7oC).

Violation: ICE BLOCKS WERE STORED IN DIRECT CONTACT WITH CONTAINERS OF ICE CREAM.
Recommendation: Nonfood items such as ice blocks, flowers, and carvings shall be stored in a designated area separate from food items. Separation may be accomplished by using separate storage racks, physical distance, divider walls or partitions or separate refrigeration units or rooms.

Violation: THE DOUBLE DOOR CABINETS USED TO STORE UNWRAPPED BREAD WERE TO FULL TO ALLOW THE DOORS TO CLOSE COMPLETELY. THIS SUBJECTED THE BREAD TO CONTAMINATION FROM AN AIR SUPPLY DUCT LOCATED IN THE DECKHEAD CLOSE BY.
Recommendation: Ensure that the cabinets are not so full that they cannot be closed completely.

Violation: THE ICE SCOOP TO THE CRUSHED ICE MACHINE WAS STORED ON TOP OF THE DOOR.
Recommendation: Between uses, ice transfer receptacles shall be stored in a way that protects them from contamination.

Violation: THE GRILL GREASE PAN WAS LOCATED DIRECTLY BEHIND THE VERTICAL FIRE DOOR SUPPORT MAKING IT IMPOSSIBLE TO REMOVE AND DIFFICULT TO CLEAN.
Recommendation: Modify the drip pan to allow removal for cleaning.

Site: Other-MAIN GALLEY / CUP HEATERS
Violation: A PAIR OF LEATHER WORK GLOVED, USED TO HANDLE THE HEATED CUPS WERE FOUND ON TOP OF ONE OF THE UNITS. THESE GLOVES ARE NOT EASY TO CLEAN.
Recommendation: Provide either gloves that are capable of being laundered or plasticized gloves that can be washed, rinsed and sanitized.

Violation: ALUMINUM FOIL WAS FOUND IN A CARDBOARD BOX ON THE PREP COUNTER CARDBOARD IS NOT EASY TO CLEAN.
Recommendation: Surfaces of equipment not intended for contact with food, but which are exposed to splash or food debris or which otherwise require frequent cleaning, shall be designed and fabricated to be smooth, washable, free of unnecessary ledges, projections, or crevices, and readily accessible for cleaning, and shall be of such material and in such repair as to be easily maintained in a clean and sanitary condition.

Site: Other-CREW MESS
Violation: IT TOOK AT LEAST 5 RUNS WITH A HOLDING TEMPERATURE THERMOMETER TO ACHIEVE A FINAL RINSE TEMPERATURE OF AT LEAST 165oF. IT APPEARED THAT THE PROBLEM WAS THE FINAL RINSE DID NOT TRIGGER UNTIL THE DISH RACK WAS 1/3 OF THE WAY THROUGH THE FINAL RINSE CHAMBER.
Recommendation: Dishes and utensils must be cleaned and sanitized utilizing spray-type dishwashing machines. These machines shall be properly installed and maintained in good repair. Machines shall be operated in accordance with manufacturers' instructions, and utensils and equipment placed in the machine shall be exposed to all dishwashing cycles. All automatic detergent dispensers, wetting agent dispensers, and liquid sanitizer injectors shall be properly installed and maintained.

Site: Other-MAIN GALLEY / BREAKFAST PANTRY
Violation: A SANITIZE BUCKET WAS FOUND IN THIS AREA WITHOUT A CHLORINE RESIDUAL.
Recommendation: Rinsing, spraying, or swabbing with a chemical sanitizing solution of at least 100 ppm chlorine or 25 ppm iodine must be used on equipment too large to be sanitized by immersion. Such solutions shall not exceed 200 ppm chlorine or 50 ppm iodine.

Violation: THE BACKSIDES OF THE GASKETS ON THE SOFT SERVE MACHINES WERE SOILED WITH FOOD RESIDUE.
Recommendation: Food shall be prepared on surfaces that prior to use have been cleaned, rinsed and sanitized.

Violation: A LEXAN CONTAINER OF SERVING BASKETS WAS FOUND SOILED WITH FOOD DEBRIS AND DUST. THE BASKETS WERE ALSO SOILED.
Recommendation: To prevent cross-contamination, pots, pans, utensils and other food contact surfaces of equipment shall be thoroughly washed, rinsed and sanitized after each use and prior to storage. Any items which are found to be soiled after being washed, must be returned to the warewashing area for immediate re-washing.

Violation: THE CLEAN FLATWARE AND DISH TROLLEY WAS LOCATED DIRECTLY UNDER AN INSECT ATTRACTION LIGHT, SUBJECTING THE CLEAN ITEMS TO CONTAMINATION FROM THE UNIT.
Recommendation: The trolley was moved to a more sanitary location during the inspection.

Violation: THIS ARE WAS VERY HOT AND HUMID.
Recommendation: Ventilation systems shall be adequately designed to remove a large percentage of the hot humid air in food service areas to prevent workers from perspiring profusely and to prevent the accumulation of condensation on deckheads.
