Violation: DURING PROVISIONING POTENTIALLY HAZARDOUS FOODS, SUCH AS DAIRY PRODUCTS, AWAITING DELIVERY TO THE GALLEY WERE PLACED IN THE FRESH FRUIT WALK-IN ON DECK 3. THIS UNIT WAS NOT ON WHEN ENTERED DUE TO PROVISIONING. THE STAFF STATED THAT THE UNIT WAS IN A DEFROST CYCLE. THE AMOUNT OF TIME THESE ITEMS WERE TO BE STORED PRIOR TO DELIVERY WAS APPROXIMATELY 3 HOURS. THE ITEMS WERE IMMEDIATELY MOVED TO THE BLAST CHILLERS IN THE GALLEYS.
Recommendation: Ensure that potentially hazardous foods are in refrigeration units that are running even during provisioning. It may be helpful to deliver these items early in the morning on provisioning days rather than holding them until 2 p.m.

Violation: THERE WERE SEVERAL DATA ELEMENTS MISSING FROM THE COMPUTERIZED GASTROINTESTINAL ILLNESS LOG. SOME OF THESE ITEMS WERE SEX, MEAL SEATING, AND NUMBER OF EPISODES OF DIARRHEA AND VOMITING.
Recommendation: 4.1.2.1.4 The gastrointestinal illness surveillance log entry for each passenger or crew member shall contain the following information: (1) The first date of clinic visit or report to staff of illness; (2) The person's name, age and gender; (3) A designation as passenger or crew member; (4) Crew member position or job on the vessel, if applicable; (5) Cabin number; (6) Meal seating information; (7) Date and time of illness onset; (8) Illness symptoms, including the presence of the following selected signs and symptoms: numbers of episodes each of diarrhea and vomiting per day, bloody stools, fever, recorded temperature; (9) Notation on whether or not a stool specimen was requested and received; (10) Use of antidiarrheal medication; and (11) The presence of underlying medical conditions which may affect interpretation of acute gastrointestinal illness for example diabetic diarrhea, inflammatory bowel disease, gastrectomy or others.

Recommendation: The reduced pressure valve assembles should have a valve on both sides of the unit.

Violation: THE POTABLE WATER LINES THAT SERVE THE HANDWASH STATION IN THE GARBAGE ROOM WERE NOT MARKED WITH A BLUE STRIP.
Recommendation: 5.3.2.1.1 Potable water piping shall be painted light blue or striped with 15 cm (6 inches) light blue bands or a light blue stripe at fittings on each side of partitions, decks, and bulkheads and at intervals not to exceed 5 m (15 feet) in all spaces, except where the decor would be marred by such markings.

Recommendation: 7.3.2.3.2 Shellstock shellfish tags shall: (1) Remain attached to the container in which the shellstock are received until the container is empty. (2) Be maintained by retaining shellstock tags or labels for 90 calendar days from the date the container is emptied by using an approved record keeping system that keeps the tags or labels in chronological order correlated to the date when the shellstock are served.

Violation: SMOKED SALMON HAD A PRODUCTION DATE OF 7/19 AND WAS TO BE USED ON 7/21. THE SALMON WAS TO BE USED MORE THAN 24 HOURS AFTER OPENING BUT WAS NOT LABELED WITH THE DISCARD OR USE BY DATE.
Recommendation: 7.3.5.3.2 Refrigerated, ready-to-eat, potentially hazardous food: (2) A container of refrigerated, ready-to-eat potentially hazardous food prepared and packaged by a food processing plant shall be clearly marked, at the time the original container is opened, to indicate the date by which the food shall be consumed which is, including the day the original container is opened, 7 calendar days or fewer after the original container is opened.
