Violation: ON 4/12/04, A 24 HOUR PRIOR TO ARRIVAL GASTROINTESTINAL ILLNESS REPORT WAS SENT AT 10:31 AM FOR A 7:00 AM ARRIVAL ON 4/13/04. ON 3/27/04, A 24 HOUR PRIOR TO ARRIVAL GASTROINTESTINAL ILLNESS REPORT WAS SENT AT 11:34 AM FOR A 7:00 AM ARRIVAL ON 3/28/04. IT WAS STATED BY THE NURSE THAT SHE OFTEN HAS PROBLEMS SENDING THE REPORTS ON TIME AND THAT IT SEEMS TO BE A PROBLEM WITH THE SHIP'S SATELLITE. SHE ALSO STATED THAT THE COMPUTER IS SET TO SEND THE REPORT AT 3:00 OR 4:00 AM THE DAY BEFORE ARRIVAL, BUT SOMETIMES THE REPORT DOES NOT LEAVE THE SHIP'S SYSTEM UNTIL A FEW HOURS LATER.
Recommendation: 4.2.1.1.1 The master, the medical staff, or other designated staff of a vessel destined for a U.S. port from a foreign port shall submit at least one standardized gastrointestinal illness report based on the number of reportable cases in the gastrointestinal illness log to the VSP no less than 24 hours, but not more than 30 hours before the vessel's expected arrival at the U.S. port.

Recommendation: 7.3.3.5.1 Food shall be protected from contamination by storing the food: (1) In a clean, dry location; (2) Where it is not exposed to splash, dust, or other contamination; and (3) At least 15 centimeters (6 inches) above the deck.
