Recommendation: In developing the Corrective Action Statement for this inspection, critical-item deficiencies (designated with Yes in Critical Column (worth 3 - 5 points), whether debited or not, should include standard operating procedures and monitoring procedures implemented to prevent the recurrence of the critical deficiency. Prepare Corrective Action Statement as a word processing or spreadsheet file which will be sent to USPHS / VSP as an email message attachment. Site: Other-PROVISIONS GENERAL
Violation: WOODEN PALLETS OF FOOD WERE FOUND STORED IN THE PROVISIONS PASSAGEWAYS POTENTIALLY EXPOSING FOOD TO CONTAMINATION. THIS WAS MOST LIKELY DUE TO THE 14 DAY LOADING OF PROVISIONS.
Recommendation: Ensure that food provisioned is transferred to the storerooms.

Violation: NUMEROUS BOXES OF LIQUOR WERE FOUND SETTING DIRECTLY ON THE DECK.
Recommendation: 7.3.3.5.1 Food shall be protected from contamination by storing the food: (1) In a clean, dry location; (2) Where it is not exposed to splash, dust, or other contamination; and (3) At least 15 centimeters (6 inches) above the deck.

Violation: THERE WAS AN EXTENSION CORD LAYING DIRECTLY ON THE BEVERAGE COUNTER MAKING THE AREA DIFFICULT TO CLEAN.
Recommendation: Install additional power outlets or raise the cord off of the counter.

Violation: DIFFICULT TO CLEAN WOODEN PALLETS WHERE FOUND IN NUMEROUS COLD ROOMS.
Recommendation: Ensure that food items are transferred from the wooden shipping pallets to easy to clean storage pallets in the cold rooms

Violation: THE EMBER GLO GRILL GREASE PAN HOUSING WAS A DIFFICULT TO CLEAN, DEEP, OPEN CHAMBER.
Recommendation: 7.4.2.5.2 Nonfood-contact surfaces of food equipment shall comply with American National Standards Institute (ANSI) or other internationally accredited food-equipment sanitation standards for materials, design, and construction. Significant modification is needed to make the housing area for the two grease collection pans in this unit easy to clean.

Violation: A LARGE BIN OF SOILED DISHWARE WAS STORED DIRECTLY ON THE DECK AT THE GLASSWASH MACHINE. ANOTHER BIN FULL OF SOILED DISHWARE WAS STORED ON A DISHWARE RACK WHICH WAS SET ON THE DECK.
Recommendation: 7.5.3.2.1 Drainboards, utensil racks, or tables large enough to accommodate all soiled and cleaned items that may accumulate during hours of operation shall be provided for necessary utensil holding before cleaning and after sanitizing.

Site: Other-MAIN GALLEY / POTWASH
Violation: THE MOUNTED THERMOMETER ON THE SANITIZE SINK WAS OUT OF CALIBRATION BY 19F. THE MANUAL READING WAS 183F AND THE MOUNTED THERMOMETER WAS READING 164F. THE UTILITIES WORKING IN THE AREA DID NOT HAVE A THERMOMETER BUT HAD TO RELY ON THE CHIEF STEWARD TO CHECK THE TEMPERATURES.
Recommendation: 7.5.2.1.1 Water temperature measuring devices: (1) That are scaled in Celsius or dually scaled in Celsius and Fahrenheit shall be designed to be accurate to 1.5C in the intended range of use. (2) That are scaled only in Fahrenheit shall designed to be accurate to 3F in the intended range of use. Recommend providing the utility with a thermometer so he can check the temperature of the sanitize sink.

Violation: WHEN TRAYS OF PLASTIC CUPS WERE RUN THROUGH THE CONVEYOR GLASSWASH MACHINE A SECOND TRAY WAS INVERTED AND PLACED ON TOP TO RESTRAIN THE CUPS. THE TOP TRAY WAS A TYPE WITH SMALL HOLES WHICH DID NOT ALLOW WATER FROM ABOVE TO CONTACT THE CUPS.
Recommendation: Ensure that if a second rack must be set on the cups to hold in-place, that the upper rack allows water entry freely to cover the top surface of the cups.

Violation: SEVERAL PREVIOUSLY CLEANED BAIN MARIE PANS WERE SOILED WITH A MILKY WATER SUBSTANCE IN CLEAN STORAGE.
Recommendation: 7.5.5.1.1 Food-contact surfaces of equipment and utensils shall be clean to sight and touch.

Violation: THE GRILL GREASE PAN HOUSING ON THE EMBER GLO GRILL WAS SOILED WITH OLD GREASE RESIDUE. THIS UNIT WAS POSTED OUT OF SERVICE.
Recommendation: 7.5.5.1.3 Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.

Violation: THE DECORATIVE WOOD AND STYROFOAM SIGNS HANGING FROM FRONT COUNTER DECKHEADS OR POSITIONED ON THE COUNTERTOP SHELF WERE FOUND SOILED WITH FOOD RESIDUE AND DUST DEBRIS AT NEARLY EVERY PANTRY, INCLUDING DELI, ROTISSERIE, AND GRILL AREAS.
Recommendation: Each such sign shall be nonabsorbent and easy to clean. Each sign should be cleaned on a daily basis to prevent such food and dirt debris accumulations. 7.5.5.1.3 Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.

Violation: THE OVER SHELF ABOVE THE COLD WELLS WHERE THE PLATE COVERS WERE STORED WAS FOUND SOILED WITH FOOD DEBRIS.
Recommendation: 7.5.5.1.3 Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.

Violation: THE MOUNTED PAPER TOWEL DISPENSER AT THE HANDWASH STATION IN THE FRONT BAR WAS POSITIONED DIRECTLY OVER THE SALAD PREPARATION COUNTER.
Recommendation: Move the paper towel holder to a position on the bulkhead to the right of the handwash sink to prevent dripping of water onto the salad preparation counter.

Violation: THE TOILET ROOM WAS DIRTY AT THE DECK, BULKHEAD, AND TOILET SEAT. WASTE PAPER TOWELS WERE OVERFLOWING THE SMALL GARBAGE BUCKET ONTO THE DECK BELOW. THE GARBAGE BUCKET WAS NOT DESIGNED FOR GARBAGE WASTE AND SHOULD BE REPLACED.
Recommendation: 7.7.1.3.2 Handwashing facilities shall be kept clean and in good repair. Provide a proper waste receptacle in the men's toilet with sufficient holding capacity to prevent waste towels from piling on the deck.

Site: Other-LIDO - CORRIDORS
Violation: THE CORRIDOR FROM THE FOOD ELEVATOR LOBBY TO EACH PANTRY AND GALLEY ENTRYWAY WAS A SHEET VINYL PRODUCT WHICH WAS SEVERELY DAMAGED IN AREAS. BULKHEAD PANELS WERE DENTED SEVERELY IN AREAS AND DECK MOLDING TRIM WAS BROKEN IN AREAS. THE DECK/BULKHEAD JUNCTURE WAS NOT COVED.
Recommendation: 7.7.4.1.1 Decks, bulkheads, and deckheads in food preparation, warewashing, pantries, and storage areas shall be constructed and maintained for easy cleaning. 7.7.4.1.3 Bulkhead and deck junctures shall be coved. Repairs are needed now for this area, but if replacement occurs the deck must be a hard (tiles, poured epoxy, or painted steel). Vinyl sheeting is not permitted in this transport route.

Violation: THE DECKHEAD THROUGHOUT THIS GALLEY SPACE HAD A HEAVY DUST RESIDUE.
Recommendation: 7.7.4.2.1 Decks, bulkheads, deckheads, and attached equipment in food preparation, warewashing, pantries, and storage areas, shall be cleaned as often as necessary.

Violation: THERE WERE SEVERAL AREAS OF THE DECK IN THIS PANTRY WHERE THE GROUTING WAS MISSING.
Recommendation: 7.7.4.2.3 Decks, bulkheads, and deckheads in food preparation, warewashing, pantries, and storage areas, shall be maintained in good repair.

Violation: ARTIFICIAL LIGHT LEVELS AT THE TWO PREPARATION COUNTERS WERE BELOW THE MINIMUM 220 LUX.
Recommendation: 7.7.5.1.1 The light intensity shall be at least 220 Lux (20 foot candles) on food preparation surfaces, and at a distance of 75 centimeters (30 inches) above the deck in food preparation areas, handwashing facilities, warewashing areas, equipment, and utensil storage, pantries, toilet rooms, and consumer self-service areas.

Violation: THE ATTEMPTED VIEWING GLASS FOR THE CONDENSATE COLLECTION PAN WAS ONLY PARTIALLY SUCCESSFUL. DURING THE INSPECTION A FIBER OPTIC DEVICE WAS USED TO VIEW INSIDE THE DRAIN PAN OF ONE FAN UNIT AND WAS EFFECTIVE.
Recommendation: Ensure that staff are trained in the use of this device and can use it to view the condensate collection pan condition and cleanliness on future operational inspections.
