Violation: THE MOST RECENT PORT MICROBIOLOGICAL REPORT FROM NEW YORK WAS DATED AUGUST 2001. THE MOST RECENT PORT MICROBIOLOGICAL REPORT FROM ST. JOHN, CANADA WAS DATED SEPTEMBER 2003.
Recommendation: 5.1.1.2.1 Where available, the vessel shall have a copy of the most recent microbiologic report from each port before bunkering potable water to verify that the water meets potable standards. Ensure that reports are not more than 30 days old.

Violation: A FEW OF THE CHAINS ON THE POTABLE WATER FITTING CAPS WERE CORRODED IN THE AFT STARBOARD AND AFT PORT BUNKER STATIONS.
Recommendation: 5.3.2.1.4 The potable water filling line shall have a screw cap or plug fastened by a non-corroding chain to an adjacent bulkhead or surface in such a manner that the cap or plug shall not touch the deck when hanging free.

Violation: THE PROCEDURE FOR THE POTABLE WATER TANK CLEANING AND DISINFECTION WAS NOT WRITTEN IN THE LOG BOOK.
Recommendation: 5.3.4.1.4 Disinfection following potential contamination shall be accomplished by increasing free residual halogen to at least 50 mg/L (ppm) throughout the affected area and maintaining this concentration for 4 hours. 5.3.4.1.3 Documentation of the cleaning shall be maintained for 12 months and shall be available to the VSP for review during inspections.

Violation: THE TEST RESULTS SHOWING THE PRESSURE DIFFERENCES FOR THE INTERNATIONAL SHORE CONNECTIONS WERE NOT RECORDED IN THE LOG BOOK.
Recommendation: 5.7.2.2.2 Backflow prevention devices requiring testing, for example reduced pressure backflow preventer and double check valves with test cocks, shall be inspected and tested with a test kit at least annually. Test results showing the pressure differences on both sides of the valves shall be maintained for each device.

Violation: PLASTIC WRAP WAS STORED ON A SHELF WITH PAPERWORK IN A CABINET. WHEN QUESTIONED HOW THE PLASTIC WRAP WAS USED THE CREW MEMBER STATED THAT SOME FOODS, AS OKAYED BY CHEF, WERE REMOVED FROM THE BUFFET LINE FOR RE-USE AT ANOTHER SERVICE LINE. THE LIDO WAS MENTIONED AS THE OTHER SERVICE LINE.
Recommendation: 7.3.3.6.4 After being served and in the possession of a consumer, food that is unused or returned by the consumer: (1) May not be offered as food for human consumption. Foods on a buffet, that are not individually packaged, are considered to be in the possession of a consumer and may not be re-used.

Violation: THE CHEF STATED THAT TEMPERATURE CONTROL WAS USED ON THE SERVICE LINE TO INCLUDE THE COLD DESSERT. THE HOT FOODS TESTED WERE AT OR ABOVE 140°F. THE PARFAIT WAS TESTED AT 56°F. THE BACK-UP FOR THIS ITEM WAS TESTED AT 40°F. AFTER THE TEMPERATURES WERE TAKEN THE CHEF STATED THAT THE DESSERT WAS ON TIME CONTROL AND WHEN ASKED A TIME CONTROL LOG WAS BROUGHT FROM THE GALLEY THAT LISTED A CAKE DESSERT.
Recommendation: 7.3.5.3.5 If time only, rather than time in conjunction with temperature, is used as the public health control for a working supply of potentially hazardous food before cooking, or for ready-to-eat potentially hazardous food that is displayed or held for service for immediate consumption: (1) The food shall be marked or otherwise identified to indicate the time that is 4 hours past the point in time when the food is removed from temperature control; (2) The food shall be cooked and served, served if ready-to-eat, or discarded, within 4 hours from the time when the food is removed from temperature control; and (3) The food in unmarked containers or packages or marked to exceed a 4 hour limit shall be discarded. 7.3.5.3.6 Written procedures that ensure compliance with these guidelines shall be maintained on the vessel and made available to the VSP, upon request.
