Violation: FOR THE 11/7-14 CRUISE, A PASSENGER WAS SEEN AFTER THE 24 HOUR PRIOR TO ARRIVAL GASTROINTESTINAL ILLNESS REPORT WAS SENT. A 4 HOUR PRIOR TO ARRIVAL REPORT WAS NOT SENT TO UPDATE THE NUMBER OF ILL PASSENGERS.
Recommendation: 4.2.1.1.2 If the number of cases changes after submission of the initial report, an updated report shall be submitted no less than 4 hours before the vessel's arrival at the U.S. port.

Violation: FOR THE 10/24-31 CRUISE, AN ILL CREW MEMBER'S POSITION WAS NOT RECORDED IN THE LOG BOOK.
Recommendation: 4.1.2.1.4 The gastrointestinal illness surveillance log entry for each passenger or crew member shall contain the following information: (1) The first date of clinic visit or report to staff of illness; (2) The person's name, age and gender; (3) A designation as passenger or crew member; (4) Crew member position or job on the vessel, if applicable; (5) Cabin number; (6) Meal seating information; (7) Date and time of illness onset; (8) Illness symptoms, including the presence of the following selected signs and symptoms: numbers of episodes each of diarrhea and vomiting per day, bloody stools, fever, recorded temperature; (9) Notation on whether or not a stool specimen was requested and received; (10) Use of antidiarrheal medication; and (11) The presence of underlying medical conditions which may affect interpretation of acute gastrointestinal illness for example diabetic diarrhea, inflammatory bowel disease, gastrectomy or others.

Violation: ON 06/27/2004 BUNKERING BEGAN AT 06:50 AND THE FIRST TIME THE FREE CHLORINE RESIDUAL WAS NOTED TO BE AT LEAST 2.0 PPM WAS 08:00. ON 07/04/2004 BUNKERING BEGAN AT 07:15 AND THE FIRST TIME THE FREE CHLORINE RESIDUAL WAS NOTED TO BE AT LEAST 2.0 PPM WAS 09:00.
Recommendation: 5.2.1.1.1 Potable water shall be continuously halogenated to at least 2.0 mg/L (ppm) free residual halogen at the time of bunkering or production with an automatic halogenation device.

Violation: THE BACK-UP CHLORINATION PUMPS FOR THE UPPER AND LOWER LOOPS OF THE DISTRIBUTION SYSTEM WERE NOT SET TO SWITCH ON AUTOMATICALLY SHOULD THE PRIMARY PUMPS FAIL.
Recommendation: 5.4.1.2.3 At least one backup halogen pump shall be available with automatic switchover to maintain the free residual halogen in the event that the primary pump fails.

Violation: THE PRESSURE DIFFERENTIALS WERE NOT CONSISTENTLY RECORDED FOR THE TESTABLE BACKFLOW PREVENTION DEVICES.
Recommendation: 5.7.2.2.2 Backflow prevention devices requiring testing, for example reduced pressure backflow preventer and double check valves with test cocks, shall be inspected and tested with a test kit at least annually. Test results showing the pressure differences on both sides of the valves shall be maintained for each device.

Violation: THERE WERE SEVERAL AREAS WHERE THE SMALL DIAMETER POTABLE WATER LINES WERE NOT STRIPED BLUE. THESE AREAS WERE AS FOLLOWS: POOL/SPA TECHNICAL ROOMS, FAN ROOMS, PHOTO LAB, AND GARBAGE ROOM.
Recommendation: 5.3.2.1.1 Potable water piping shall be painted light blue or striped with 15 cm (6 inches) light blue bands or a light blue stripe at fittings on each side of partitions, decks, and bulkheads and at intervals not to exceed 5 m (15 feet) in all spaces, except where the decor would be marred by such markings.

Site: Other-WINDJAMMER BEVERAGE STATIONS
Violation: BACKFLOW PREVENTION DEVICES WERE NOT INSTALLED ON THE WATER LINES SUPPLYING THE COFFEE MACHINES.
Recommendation: Install the appropriated vented backflow prevention devices on these lines. 5.7.1.1.3 The vessel shall provide a comprehensive cross- connection control program that provides safe connections to the potable water system through air gaps or appropriate backflow devices at the following locations, if present: (1) Potable water supply lines to swimming pools, whirlpool spas, hot tubs, bathtubs, showers, and similar facilities; (2) Photographic laboratory developing machines and utility sinks; (3) Beauty and barber shop spray-rinse hoses; (4) Potable water faucets where hoses are connected or can be connected by threaded or quick-connect outlets such as those serving tanks containing chlorine and other chemicals, and deck taps; (5) Garbage grinders and pulpers; (6) Mechanical warewashing machines; (7) Hospital and laundry equipment; (8) Air conditioning expansion tanks; (9) Boiler feed water tanks; (10) Fire systems; (11) Toilets; (12) Potable water, bilge, and sanitary pumps that require priming; (13) Freshwater or saltwater ballast systems; (14) Bilge or other waste water locations; (15) International shore connection; and (16) Any other connection between potable and non-potable water systems.

Violation: THE FREE CHLORINE RESIDUAL WAS TESTED AT 0.12 PPM. A BROWN "ORGANIC" MATERIAL WAS NOTED FLOATING AT VARIOUS SECTIONS OF THE POOL. IT COULD NOT BE DETERMINED WHAT THIS MATERIAL WAS. THE POOL WAS IMMEDIATELY CLOSED.
Recommendation: 6.1.1.2.3 No bathers shall be allowed to use the pool before a free residual halogen of 1.0 mg/L (ppm) is achieved. 6.2.2.1.1 A free residual halogen of >1.0 and <3.0 mg/L (ppm) shall be maintained in recirculated swimming pools.

Violation: FREE BROMINE RESIDUALS IN EXCESS OF 10 PPM WERE NOTED IN THE FOLLOWING WHIRLPOOL SPAS: WHIRLPOOL 6- 12 PPM WHIRLPOOL 4- 15 PPM WHIRLPOOL 3- 16 PPM THE WHIRLPOOLS WERE IMMEDIATELY CLOSED.
Recommendation: 6.3.2.1.1 Whirlpool spas shall maintain a free residual chlorine of >3.0 mg/L (ppm) and <10 mg/L (ppm), or a free residual bromine of >4.0 mg/L (ppm) and <10 mg/L (ppm).

Violation: THE KID'S POOL WAS SAID TO BE CLOSED. IT WAS NOTED THAT THE SCALLOP SHAPED POOL WAS NOT NETTED AND WAS FILLED WITH WATER. THE CIRCULAR POOL WAS NETTED. THE BROMINE RESIDUAL AT THE OPEN POOL WAS 0 PPM FREE BROMINE. A CHILD WAS USING THIS POOL AT THE TIME OF THE INSPECTION.
Recommendation: Ensure that closed pools are netted or empty. 6.1.1.2.3 No bathers shall be allowed to use the pool before a free residual halogen of 1.0 mg/L (ppm) is achieved. 6.2.2.1.1 A free residual halogen of >1.0 and <3.0 mg/L (ppm) shall be maintained in recirculated swimming pools.

Violation: THE SCALLOP SHAPED KID'S POOL RECIRCULATION SYSTEM HAD BEEN SHUT OFF DUE TO A LEAKING PUMP. THE POOL WAS SAID NOT TO BE IN USE BUT DURING THE INSPECTION IT WAS NOTED TO BE IN USE.
Recommendation: 6.1.2.1.1 Re-circulated swimming pool water shall be filtered.

Violation: ANTI-VORTEX DRAIN COVERS WERE NOT PROVIDED IN THE KID'S POOLS.
Recommendation: 6.4.1.2.2 Anti-vortex drain covers shall be provided on swimming pools and whirlpool spas.

Violation: A CORE SAMPLE WAS NOT USED WHEN CONDUCTING THE MONTHLY SEDIMENTATION TESTS.
Recommendation: 6.3.1.1.5 The granular filters shall be opened at least monthly and examined for cracks, mounds, or holes in the filter media. A core sample of the filter media shall be inspected for excessive organic material accumulation using a recommended sedimentation method.

Violation: THE SHEPHERD'S HOOK WAS NOT PLACED IN A PROMINENT LOCATION.
Recommendation: 6.4.1.2.1 Easy access shepherd's hook and approved floatation device shall be provided at a prominent location near each public swimming pool.

Violation: HAMS IN THE HOT HOLD CABINETS WERE TESTED AT 115°F AND 117°F. THE FOODS IN BACK-UP HOT HOLD UNITS WERE SAID TO BE ON TEMPERATURE CONTROL. THE HAMS WERE SAID TO HAVE BEEN PLACED IN THE HOT HOLD UNITS 1 HOUR PRIOR. FOR THIS SERVICE THE HAMS WERE PLACED ON TIME CONTROL.
Recommendation: 7.3.5.3.1.1 Potentially hazardous food shall be maintained: (1) At 60°C (140°F) [roasts 54°C (130°F)] or above,; or (2) At 5°C (41°F) or less.

Violation: CREAM IN THE UNDERCOUNTER REFRIGERATION UNIT WAS TESTED AT 45°F. THE THERMOMETER IN THIS UNIT NOTED 46°F.
Recommendation: 7.3.5.3.1.1 Potentially hazardous food shall be maintained: (1) At 60°C (140°F) [roasts 54°C (130°F)] or above,; or (2) At 5°C (41°F) or less.

Site: Other-WINDJAMMER BUFFET
Violation: THE CUT MELON WAS PREPARED FOR SEVERAL BUFFET STATIONS THAT HAD STAGGERED OPERATING HOURS. A REVIEW OF THE TIME CONTROL PLAN AND THE OPERATING HOURS OF THE BUFFETS INDICATED THAT IT WAS POSSIBLE FOR THE CUT MELONS TO BE ON TIME CONTROL FOR MORE THAN 4 HOURS. THE BULK PANS OF CUT MELONS WERE NOT LABELED WITH THE 4-HOUR DISCARD TIME. THE SET-UP AND DISCARD TIMES FOR THE BUFFET STATIONS WERE NOT INDICATED ON THE TIME CONTROL PLAN.
Recommendation: Label the bulk pans of melons with the discard time. Include the set-up and discard times for each buffet station on the time control plan. 7.3.5.3.5 If time only, rather than time in conjunction with temperature, is used as the public health control for a working supply of potentially hazardous food before cooking, or for ready-to-eat potentially hazardous food that is displayed or held for service for immediate consumption: (1) The food shall be marked or otherwise identified to indicate the time that is 4 hours past the point in time when the food is removed from temperature control; (2) The food shall be cooked and served, served if ready-to-eat, or discarded, within 4 hours from the time when the food is removed from temperature control; and (3) The food in unmarked containers or packages or marked to exceed a 4 hour limit shall be discarded.

Violation: GROUND BEEF WAS STORED ABOVE PRIMAL CUTS OF BEEF IN THE WALK-IN COOLER.
Recommendation: 7.3.3.2.1.1 Food shall be protected from cross-contamination by: (2) Separating types of raw animal foods from each other such as beef, fish, lamb, pork, and poultry during storage, preparation, holding, and display by using separate equipment for each type, or arranging each type of food in equipment so that cross-contamination of one type with another is prevented, and preparing each type of food at different times or in separate areas.

Site: Other-WINDJAMMER/ISLAND GRILL BUFFETS
Violation: THE SIDES OF THE BUFFETS WERE NOT ADEQUATELY PROTECTED. THIS APPLIED TO BOTH THE PERMANENT AND PORTABLE BUFFET STATIONS. THE VESSEL STARTED TO MAKE CORRECTIONS TO THE SIDES OF THE TEMPORARY BUFFET STATIONS DURING THE INSPECTION. A GAP WAS NOTED BETWEEN THE SECTIONS OF SNEEZE GUARD NEAR THE CARVING STATION IN THE ISLAND GRILL. THE PORT BUFFET SNEEZE GUARD WAS TOO HIGH TO ADEQUATELY PROTECT THE FRUIT. THERE WERE SEVERAL FOOD ITEMS THAT WERE NOT PLACED UNDER THE BUFFET SNEEZE GUARDS. THESE ITEMS INCLUDED THE OATMEAL/GRITS CONDIMENTS, THE SOUP STATION CONDIMENTS, AND GRAPES. SERVING UTENSILS WERE NOT PROVIDED FOR THE WHOLE FRUIT. THE UTENSILS USED FOR THE OMELET STATION INGREDIENTS HAD SHORT HANDLES. THE HANDLES WERE NOT OUTSIDE OF THE FOOD CONTAINER IN BETWEEN USE. A SERVING UTENSIL WAS NOT PROVIDED FOR EACH FOOD ITEM AT THE PORT WINDJAMMER HOT BUFFET. VERY LARGE PANS WERE USED TO SERVE BREAD AND ROLLS AT THE BUFFET BUT SHORT HANDLED TONGS WERE PROVIDED. THE HANDLES WERE NOTED TO BE IN CONTACT WITH THE BREAD.
Recommendation: 7.3.3.6.1 Food on display shall be protected from contamination by the use of packaging; counter, service line, or salad bar food guards; display cases; or other effective means. 7.3.3.6.3 Consumer self-service operations, such as salad bars and buffets, for unpackaged ready-to-eat foods: (1) Shall be provided with suitable utensils or effective dispensing methods that protect the food from contamination; and (2) Shall be monitored by food employees trained in safe operating procedures. 7.3.3.4.2.1 During pauses in food preparation or dispensing, food preparation and dispensing utensils shall be stored: (1) In the food with their handles above the top of the food and the container.

Violation: A SLOTTED FASTENER WAS NOTED IN THE FOOD CONTACT AREA OF THE FRYER, MAKING IT DIFFICULT TO CLEAN.
Recommendation: 7.4.1.1.2 Materials that are used in the construction of multiuse utensils and food-contact surfaces of equipment shall be: (1) Durable, corrosion-resistant, and nonabsorbent; (2) Sufficient in weight and thickness to withstand repeated warewashing; (3) Finished to have a smooth, easily cleanable surface; and (4) Resistant to pitting, chipping, crazing, scratching, scoring, distortion, and decomposition. 7.4.2.2.1 Multiuse food-contact surfaces shall be: (1) Smooth; (2) Free of breaks, open seams >1 mm (1/32 inch), cracks, chips, inclusions, pits, and similar imperfections; (3) Free of sharp internal angles, corners, and crevices; (4) Finished to have smooth welds and joints; and (5) Accessible for cleaning and inspection by one of the following methods without being disassembled, by disassembling without the use of tools, or by easy disassembling with the use of handheld tools commonly available to maintenance and cleaning personnel such as screwdrivers, pliers, open-end wrenches, and Allen wrenches.

Violation: PEELING SEALANT AND PEELING AND CRACKED GASKETS WERE NOTED IN THE POTATO PEELERS, MAKING CLEANING DIFFICULT.

Violation: THE INTERIOR SURFACES OF THE ICE CREAM DISPLAY CASES WERE NOT DESIGNED FOR EASY CLEANING. THE JUNCTURES OF THE WALLS AND BOTTOMS OF THE UNITS DID NOT HAVE COVED CORNERS. THE GRATES INSIDE OF THESE CASES HAD CHIPPED PAINT AND WERE CORRODED IN PLACES. THIS WAS NOTED ON THE LAST INSPECTION.

Violation: A SLOTTED FASTENER WAS NOTED IN THE FOOD CONTACT AREA OF THE FRYER. MISSING FASTENERS WERE NOTED IN THE FOOD CONTACT AND FOOD SPLASH ZONES OF THE FRYER. THESE CONDITIONS MADE CLEANING DIFFICULT.

Violation: THE GASKETS ON THE TECHNICAL PANELS OF THE TILT KETTLES WERE EXPOSED, PEELING, AND MISSING IN SOME AREAS, MAKING CLEANING DIFFICULT. HOLES WERE NOTED ON THE HOT AND COLD WATER KNOBS ON THE TECHNICAL PANELS OF THE TILT KETTLES WHERE THE COVERS WERE MISSING, MAKING CLEANING DIFFICULT.
