ON THE CRUISE ENDING 11/19/2004, ONE PASSENGER AND ONE CREW CASE LISTED IN THE GI LOG WERE NOT ACCOUNTED FOR IN THE 4-HOUR REPORT. FOR THE CRUISE ENDING 11/11/2004, ONE ADDITIONAL CREW CASE WAS NOT ACCOUNTED FOR ON THE 4-HOUR REPORT. IN BOTH THESE CRUISES, THE CASES REPORTED TO MEDICAL PRIOR TO THE SUBMISSION OF THE 4-HOUR REPORTS. THE 4-HOUR PRIOR TO ARRIVAL GI ILLNESS REPORTS FOR THE CRUISES ENDING 11/19/2004 AND 10/22/2004, WERE SUBMITTED LESS THAN 4-HOURS BEFORE THE VESSEL'S EXPECTED TIME OF ARRIVAL AT THE U.S. PORT.

FOR THE CRUISE ENDING 10/22/2004, SOME OF THE REQUIRED ELEMENTS OF THE GI LOG WERE NOT COMPLETED FOR SOME CASE-PATIENT ENTRIES.
