Violation: THE 4-HOUR ROUTINE GI REPORT CASE COUNTS WERE REPORTED AS THE ADDITIONAL (NEW) CASES ONLY.
Recommendation: 4.2.1.2.1 The gastrointestinal illness report shall contain: (1) The name of the vessel; (2) The ports of embarkation and disembarkation; (3) The dates of embarkation and disembarkation; (4) The total numbers of reportable cases of gastrointestinal illness among passengers and crew members, including those who have been disembarked or removed because of illness, even if the number is 0; and (5) The total number of passengers and crew members on the cruise.

Violation: A REVIEW OF THE FAR POINT CHLORINE ANALYZER CHARTS NOTED THAT THE CHARTS FOR 09/27/04 AND 09/30/04 DID NOT HAVE CALIBRATION NOTED ON THEM. THIS WAS NOTED FOR BOTH THE UPPER AND LOWER LOOPS.
Recommendation: 5.5.1.2.2 The calibration shall be recorded on the chart or in a log book.

Violation: THERE WAS NO BACK-UP CHLORINE PUMP FOR THE UPPER LOOP AND THE BACK-UP PUMP FOR THE LOWER LOOP WAS NOT PLUGGED INTO THE ELECTRICAL OUTLET.
Recommendation: 5.4.1.2.3 At least one backup halogen pump shall be available with automatic switchover to maintain the free residual halogen in the event that the primary pump fails.

Violation: THE METHOD FOR DISINFECTING POTABLE WATER TANKS WAS TO ROLL ON A 100 PPM CHLORINE SOLUTION. THERE WAS NO INDICATION FROM THE RECORDS THAT THE CONTACT TIME FOR THIS METHOD WAS SUFFICIENT.
Recommendation: Include contact time for disnfectants used on potable water tanks.

Violation: POTABLE WATER TANKS 4 P & S, 5 P & S, AND 6 WERE DISINFECTED WITH 100 PPM AFTER MAINTENANCE DURING 2004 BUT THE CONTACT TIME AND FLUSHING OF THE TANKS WAS NOT NOTED IN THE RECORDS. WORK WAS DONE ON TANK 2 P ON 06/07/2003, BUT THERE WERE NO DETAILS REGARDING THE CLEANING, DISINFECTION, AND FLUSHING OF THE TANK. WORK WAS DONE ON TANK 2 S ON 06/18/2003, BUT THERE WERE NO DETAILS REGARDING THE CLEANING, DISINFECTION, AND FLUSHING OF THE TANK. WORK WAS DONE ON TANKS 1 P AND 1 S ON 08/22/2003, BUT THERE WERE NO DETAILS REGARDING THE CLEANING, DISINFECTION, AND FLUSHING OF THE TANKS.
Recommendation: 5.3.4.1.3 Documentation of the cleaning shall be maintained for 12 months and shall be available to the VSP for review during inspections. Document the contact time and concentration of disinfectant used on potable water tanks.

Site: Other-POTABLE WATER
Violation: RECORDS INDICATED THAT THE REDUCED PRESSURE DEVICES ON THE INTERNATIONAL SHORE CONNECTIONS WERE INSPECTED BUT NOT TESTED AS REQUIRED.
Recommendation: 5.7.2.2.2 Backflow prevention devices requiring testing, for example reduced pressure backflow preventer and double check valves with test cocks, shall be inspected and tested with a test kit at least annually. Test results showing the pressure differences on both sides of the valves shall be maintained for each device.

Violation: THE POTABLE WATER LINE FROM THE BUNKER STATION, IN THE ENGINE ROOM, WAS NOT STRIPED BLUE BUT WAS INSTEAD STRIPED GREEN.
Recommendation: 5.3.2.1.1 Potable water piping shall be painted light blue or striped with 15 cm (6 inches) light blue bands or a light blue stripe at fittings on each side of partitions, decks, and bulkheads and at intervals not to exceed 5 m (15 feet) in all spaces, except where the decor would be marred by such markings.

Violation: A STEADY LEAK WAS NOTED AT THE BACKFLOW PREVENTION DEVICE IN THE HOOD CLEANING CABINET.
Recommendation: Repair or replace the defective backflow prevention device.

Site: Other-WHIRLPOOL SPAS
Violation: THE ENGINEER INDICATED THAT THE WHIRLPOOL FILTERS WERE BACKWASHED TWICE WHEN HE WAS ASKED HOW HE KNEW WHEN BACKWASHING WAS COMPLETED. AFTER FURTHER DISCUSSION, HE INDICATED THAT A SIGHT GLASS WAS AVAILABLE TO VIEW IF THE WATER WAS CLEAR. THE SIGHT GLASS ON THE FILTER HOUSING FOR THE OUTSIDE WHIRLPOOLS WAS HEAVILY SOILED AND THE WATER COULD NOT BE SEEN EASILY.
Recommendation: 6.3.1.1.4 Granular filters shall be backwashed at least daily until the sight glass indicates a clean flow. The uniformity of the backwash action shall be observed, where possible. Other types of filter media shall be backwashed or cleaned at the frequency specified by the manufacturer. Enusure sight glass allows for accurate observations of backwash operations.

Violation: PRIOR TO 11/30/2004, THE CHLORINE RESIDUAL IN THE POOLS WAS TESTED ONCE EVERY 4-HOUR SHIFT. IT WAS POSSIBLE GIVEN THIS FREQUENCY THAT THE TESTING COULD BE PERFORMED MORE THAN 4 HOURS APART.
Recommendation: 6.2.2.2.2 Residual halogen logs shall be maintained with residuals measured and recorded every 4 hours during operation.

Violation: THE CHARTS FOR 08/14-27/04 WERE NOT DATED. THE CHART FOR 08/06/2004 WAS MISSING AND THE MANUAL TESTS WERE PERFORMED EVERY 2 HOURS FROM 12:00-18:00 AND NOT HOURLY AS REQUIRED.
Recommendation: 6.3.2.2.2 Residual halogen logs shall be maintained with residuals measured and recorded hourly during operation. 6.2.2.2.5 Halogen analyzer-chart recorder charts shall be initialed, dated, and changed daily.

Violation: WHEN ASKED WHAT WAS DONE WITH FOOD ON THE DISPLAY LINE THAT WAS LEFTOVER AFTER LUNCH, THE COOK AT THIS STATION INDICATED THAT THE ITEMS, THOUGH FEW, WERE SENT TO THE CREW GALLEY.
Recommendation: 7.3.3.6.4 After being served and in the possession of a consumer, food that is unused or returned by the consumer: (1) May not be offered as food for human consumption.

Site: Other-WINDFALL CAF GALLEY
Violation: PANS OF ROLLED AND TIED TURKEY BREASTS WERE TESTED AT 110F AND 123F IN THE HOT HOLDING UNITS. THE LOG FOR THESE ITEMS INDICATED THAT THE TEMPERATURE WAS 170F WHEN THEY WERE PICKED UP FROM THE MAIN GALLEY. ACCORDING TO THE TIME CONTROL PLAN, THE HOT HOLDING UNITS WERE UNDER TEMPERATURE CONTROL. THE STAFF WAS TOLD TO KEEP THESE ITEMS ON TIME CONTROL FOR THE DAY AND TO DISCARD THEM WITHIN 4 HOURS.
Recommendation: 7.3.5.3.5 If time only, rather than time in conjunction with temperature, is used as the public health control for a working supply of potentially hazardous food before cooking, or for ready-to-eat potentially hazardous food that is displayed or held for service for immediate consumption: (1) The food shall be marked or otherwise identified to indicate the time that is 4 hours past the point in time when the food is removed from temperature control; (2) The food shall be cooked and served, served if ready-to-eat, or discarded, within 4 hours from the time when the food is removed from temperature control; and (3) The food in unmarked containers or packages or marked to exceed a 4 hour limit shall be discarded.

Recommendation: 7.5.2.1.2 Pressure measuring devices that display the pressures in the water supply line for the fresh hot water sanitizing rinse shall have increments of 7 kilopascals (1 pounds per square inch) or smaller and shall be accurate to 14 kilopascals ( 2 pounds per square inch) in the 100-170 kilopascals (15-25 pounds per square inch) range.

Recommendation: 7.3.3.4.4.2 Wiping cloths shall be restricted to the following: (2) Cloths used for wiping food spills shall be dry and used for wiping food spills from tableware and single-service articles or wet and cleaned, stored in a chemical sanitizer, and used for wiping spills from food-contact and nonfood-contact surfaces of equipment.

Recommendation: 7.5.7.3.1 Cleaned equipment and utensils, laundered linens, and single-service and single-use articles shall be stored: (1) In a clean, dry location; (2) Where they are not exposed to splash, dust, or other contamination; and (3) At least 15 centimeters (6 inches) above the deck.

Recommendation: 7.7.4.2.3 Decks, bulkheads, and deckheads in food preparation, warewashing, pantries, and storage areas, shall be maintained in good repair.

Violation: THE DECK THROUGHOUT THE GALLEYS, PANTRIES, AND WAREWASH AREAS WERE IN SEVERE DISREPAIR. MISSING AND CRACKED DECK TILES WERE NOTED. THE DECKS WERE SOILED ALONG THE BULKHEAD-DECK JUNCTURES AND IN THE CORNERS. THE GROUT WAS WORN AND SOILED IN SEVERAL AREAS.
Recommendation: 7.7.4.2.3 Decks, bulkheads, and deckheads in food preparation, warewashing, pantries, and storage areas, shall be maintained in good repair.

Violation: THERE WAS NO INDICATION IN THE PLAN THAT ACTIVE MONITORING INSPECTIONS WERE CONDUCTED.
Recommendation: 8.1.1.1.3 The Integrated Pest Management Plan shall include provisions for active monitoring including pest sighting logs for the operational areas of the vessel and training for crew members in charge of log completion.
