Item No.: 3
Site: Other-POTABLE WATER SYSTEM
Violation: THE CHLORINE ANALYZER RECORDER IN USE WAS A CONTINOUS READ MODEL. A MANUAL RECORD WAS ALSO USED TO DOCUMENT FREE RESIDUAL CHLORINE WHEN THE ANALYZER WAS OUT OF SERVICE.
Recommendation: The analyzer and chart recorder must be properly maintained, operated and calibrated in accordance with the manufacturer's instructions. A review of records from the ship's automatic chlorine or bromine analyzer must show that the ship has maintained adequate amounts of free halogen residual in the water distribution system since the last inspection of the vessel. The review will be deemed unacceptable if the records show a free halogen residual of less than 0.2 PPM for more than 24 consecutive hours. Charts shall be initialed and dated daily and retained for at least 12-months. A circular chart system that records for 24 hours in a range of 0.0 PPM to 5.0 PPM should be installed on this vessel.

Site: Other-POTABLE WATER SYSTEM
Violation: THE HOSE LOCKER WAS SOILED WITH DEBRIS.
Recommendation: Potable water hoses must be flushed before the filling line is attached. Hoses must be drained after each use and stowed, with ends capped, on reels or racks in special lockers marked "POTABLE WATER HOSE AND FITTINGS STORAGE" in letters at least one-half inch high. Lockers must be closed self-draining, smooth, nontoxic, corrosion-resistant, easily cleanable, fixed 18-inches above the deck and used for no other purpose. Hoses and fittings must be maintained in good repair.

Violation: SALMON AND STEAK UNDER REFRIGERATION WERE AT 50 AND 59 DEGREES F RESPECTIVELY.
Recommendation: Potentially hazardous food requiring refrigeration should be stored at or below 45oF. Since prep and storage did not exceed 4 hours and the items were to be consumed shortly immediate cooking was initiated for both items.

Violation: THE TEMPERATURE OF THE REFRIGERATOR WAS NOT AT 40 DEGREES F. OVER STACKING OF ITEMS INHIBITED AIR FLOW.
Recommendation: Refrigerators used to store potentially hazardous foods must be maintained at 40oF (4.5oC) at all times to assure that potentially hazardous foods stored therein stabilize at or below 45oF (7oC).

Violation: TWO THERMOMETERS USED BY THE CHEF WERE OUT OF CALIBRATION.
Recommendation: Food probe thermometers must be available and used by the staff. They should be sanitized between uses. All thermometers shall be accurate to +2oF (+1oC).

Violation: NO SNEEZE GUARDS WERE USED FOR THE BUFFET SERVICE.
Recommendation: Display tables or buffet lines for service of unpackaged foods must be effectively shielded so as to intercept any source of contamination from the consumer. Appropriate food shielding (sneeze guards) should be provided.

Site: Other-MEDICAL REPORTING
Violation: SHIP REPOSITIONED FROM MEXICO AND USED AN AGENT TO REPORT GASTROINTESTINAL ILLNESSES TO VSP HOWEVER, NO DOCUMENTATION WAS PRESENT TO DEMONSTRATE THAT VSP WAS NOTIFIED ACCORDING TO PUBLISHED REQUIREMENTS.
Recommendation: Diarrhea logs in the hospital must be accurately maintained, and results must be reported to the Vessel Sanitation Program in accordance with Section III of the "Vessel Sanitation Program Operations Manual", August 1989. Further recommended direct reporting to VSP by phone or FAX. If the vessel adds a computer future reporting my also be done by email or via the internet.

Violation: SEVERAL GASKETS IN THE GALLEY AND BAR WERE CRACKED.
Recommendation: Refrigerator gaskets shall be easily cleanable without breaks in the material. The corners shall be closed and sealed and hollow sections shall be sealed.

Violation: FOOD RESIDUE WAS PRESENT ON A VARIETY OF UTENSILS.
Recommendation: All food preparation equipment, utensils, tableware, other food contact surfaces and food storage containers shall be washed, rinsed, and sanitized after every use or change in use. A food surface is that surface of equipment or utensil with which food normally comes in contact and those surfaces from which food may drip, drain, or splash back onto other surfaces normally in contact with food.

Violation: SEVERAL REFRIGERATOR GASKETS WERE FOUND SOILED WITH FOOD RESIDUE IN THE GALLEY AND BAR.
Recommendation: Non-food contact surfaces of equipment shall be cleaned as often as is necessary to keep the equipment free of accumulation of dust, dirt, food particles, and other debris.

Site: Other-PROVISIONS
Violation: THERE WERE NUMEROUS PENETRATIONS IN THE BULKHEAD OF THE PROVISIONS AREA.
Recommendation: Decks, bulkheads, and deckheads in food preparation, warewashing, and storage areas, including pantries, shall be designed and constructed for easy cleanability, shall be corrosion-resistant, maintained in good repair, and kept clean.

Violation: THE DECK WAS BUCKLING IN THE BAR AREA MAKING IT DIFFICULT TO CLEAN.
Recommendation: It is required that decks be maintained so as to be durable and easily cleanable. Coving in this are would also make cleaning easier.

Violation: THREE LIGHTS WERE OUT IN THE REACH-IN REFRIGERATOR.
Recommendation: Replace the bulbs in the reach-in refrigerator.
