Violation: THE 24-HOUR REPORTS WERE CONSISTENTLY RECEIVED LESS THAN 24 HOURS PRIOR TO ARRIVAL. ACCORDING THE MEDICAL STAFF THE SYSTEM WAS REJECTING THE REPORTS. THE REPORT FOR THIS CRUISE WAS NOTED TO MISSING THE TIME OF ARRIVAL. THIS WAS CORRECTED HOWEVER NOT USING THE 24-HOUR CLOCK. THE VESSEL DID HAVE THE REJECTION NOTICES FROM THE DATABASE SYSTEM.
Recommendation: 4.2.1.1.1 The master, the medical staff, or other designated staff of a vessel destined for a U.S. port from a foreign port shall submit at least one standardized gastrointestinal illness report based on the number of reportable cases in the gastrointestinal illness log to the VSP no less than 24 hours, but not more than 30 hours before the vessel's expected arrival at the U.S. port.

Violation: THE INSPECTION OF THE SAND FILTERS FOR CRACKS, MOUNDS, OR HOLES WAS NOT RECORDED ON A CONSISTENT BASIS.
Recommendation: 6.3.1.1.5 The granular filters shall be opened at least monthly and examined for cracks, mounds, or holes in the filter media. A core sample of the filter media shall be inspected for excessive organic material accumulation using a recommended sedimentation method.

Recommendation: 6.4.1.2.1 Easy access shepherd's hook and approved floatation device shall be provided at a prominent location near each public swimming pool.

Violation: ON 11/23/2002 AN ASSISTANT FOOD MANAGER REPORTED TO THE MEDICAL OFFICE AND WAS CLASSIFED AS A CASE OF GASTROINTESTINAL ILLNESS. ON 11/28/02 A WAITER REPORTED TO THE MEDICAL OFFICE AND WAS CLASSIFIED AS A CASE OF GASTROINTESTINAL ILLNESS. THERE WAS NO EVIDENCE THAT EITHER OF THESE INDIVIDUALS OBTAINED WRITTEN CLEARANCE PRIOR TO RETURNING TO WORK.
Recommendation: 7.2.2.1.4 The restriction may be removed when the person in charge of the food operation obtains written approval from the vessel's physician or equivalent medical staff.

Violation: THE PLATES WERE NOT ADEQUATELY SCRAPPED PRIOR TO GOING THROUGH THE DISHWASH UNIT. EXCESSIVE FOOD RESIDUE WAS NOTED ON THE PLATES. SOME PLATES STILL HAD FOOD RESIDUE COMING OUT OF THE UNIT. THE WASTE BASKET INSIDE THE UNIT HAD AN EXCESSIVE AMOUNT OF FOOD AND PAPER DEBIRS. MOUNDS OF FOOD WERE NOTED ON THE SOILED LANDING. THE PULPER SYSTEM WAS NOT IN USE.
Recommendation: 7.5.5.3.1 Food debris on equipment and utensils, shall be scrapped over a waste disposal unit, pulper, or garbage receptacle or shall be removed in a warewashing machine with a prewash cycle.

Violation: COFFEE CUPS EXITED THE DISHWASH UNIT UPRIGHT AND FILLED WITH WATER. THESE CUPS WERE NOT SENT BACK THROUGH THE MACHINE. THE LARGE WAIT TRAYS WERE LOADED INTO THE UNIT TOO CLOSE TOGETHER.
Recommendation: Rewash coffee cups when the exit the machine upright. Place the wait trays far enough apart so that the spray of the warewash unit has full contact.

Violation: THE PLATE COVERS WERE NOT STORED INVERTED. A TROLLEY OF CLEAN PLATES WAS STORED ON THE CLEAN SIDE OF THE GLASSWASH UNIT. RACKS REMOVED FROM THE GLASSWASHED UNIT DRIPPED ONTO THE PLATES IN THE TROLLEY. ONE OF THE PLATE TROLLEYS WAS SOILED.
Recommendation: 7.5.7.3.2 Clean equipment and utensils shall be stored: (1) In a self-draining position that allows air drying; and (2) Covered or inverted. 7.5.7.3.1 Cleaned equipment and utensils, laundered linens, and single-service and single-use articles shall be stored: (1) In a clean, dry location; (2) Where they are not exposed to splash, dust, or other contamination; and (3) At least 15 centimeters (6 inches) above the deck.
